Export the external file reader used in COPY FROM as APIs. They are expected to be used by extension modules like file_fdw. There are no user-visible changes. Itagaki Takahiro Reviewed and tested by Kevin Grittner and Noah Misch.

Clean up the #include mess a little. walsender.h should depend on xlog.h, not vice versa. (Actually, the inclusion was circular until a couple hours ago, which was even sillier; but Bruce broke it in the expedient rather than logically correct direction.) Because of that poor decision, plus blind application of pgrminclude, we had a situation where half the system was depending on xlog.h to include such unrelated stuff as array.h and guc.h. Clean up the header inclusion, and manually revert a lot of what pgrminclude had done so things build again. This episode reinforces my feeling that pgrminclude should not be run without adult supervision. Inclusion changes in header files in particular need to be reviewed with great care. More generally, it'd be good if we had a clearer notion of module layering to dictate which headers can sanely include which others ... but that's a big task for another day.

Add location field to DefElem Add a location field to the DefElem struct, used to parse many utility commands. Update various error messages to supply error position information. To propogate the error position information in a more systematic way, create a ParseState in standard_ProcessUtility() and pass that to interested functions implementing the utility commands. This seems better than passing the query string and then reassembling a parse state ad hoc, which violates the encapsulation of the ParseState type. pg_type has a typnamespace column; system now supports creating types in different namespaces. Also, cleanup work on relation namespace support: drop, alter, rename commands work for tables in non-default namespaces.

Add contrib/file_fdw foreign-data wrapper for reading files via COPY. This is both very useful in its own right, and an important test case for the core FDW support. This commit includes a small refactoring of copy.c to expose its option checking code as a separately callable function. The original patch submission duplicated hundreds of lines of that code, which seemed pretty unmaintainable. Shigeru Hanada, reviewed by Itagaki Takahiro and Tom Lane

Add support for piping COPY to/from an external program. This includes backend "COPY TO/FROM PROGRAM '...'" syntax, and corresponding psql \copy syntax. Like with reading/writing files, the backend version is superuser-only, and in the psql version, the program is run in the client. In the passing, the psql \copy STDIN/STDOUT syntax is subtly changed: if you the stdin/stdout is quoted, it's now interpreted as a filename. For example, "\copy foo from 'stdin'" now reads from a file called 'stdin', not from standard input. Before this, there was no way to specify a filename called stdin, stdout, pstdin or pstdout. This creates a new function in pgport, wait_result_to_str(), which can be used to convert the exit status of a process, as returned by wait(3), to a human-readable string.
