Logical decoding of TRUNCATE

Add a new WAL record type for TRUNCATE, which is only used when
wal_level >= logical.  (For physical replication, TRUNCATE is already
replicated via SMGR records.)  Add new callback for logical decoding
output plugins to receive TRUNCATE actions.
