Refactor PgFdwModifyState creation/destruction into separate functions.

Etsuro Fujita.  The larger patch series of which this is a part has
been reviewed by Amit Langote, David Fetter, Maksim Milyutin,
Álvaro Herrera, Stephen Frost, and me.
