Remove excess argument to open(2).

Many compilers don't complain about this, but some do, and it's certainly
wrong. Back-patch to 8.4 where the error was introduced.

Add $PostgreSQL$ markers to a lot of files that were missing them.

This particular batch was just for *.c and *.h file.
The changes were made with the following 2 commands:
