caddcb8f4b Fix pg_upgrade to handle extensions.
This follows my proposal of yesterday, namely that we try to recreate the
previous state of the extension exactly, instead of allowing CREATE
EXTENSION to run a SQL script that might create some entirely-incompatible
on-disk state.  In --binary-upgrade mode, pg_dump won't issue CREATE
EXTENSION at all, but instead uses a kluge function provided by
pg_upgrade_support to recreate the pg_extension row (and extension-level
pg_depend entries) without creating any member objects.  The member objects
are then restored in the same way as if they weren't members, in particular
using pg_upgrade's normal hacks to preserve OIDs that need to be preserved.
Then, for each member object, ALTER EXTENSION ADD is issued to recreate the
pg_depend entry that marks it as an extension member.

In passing, fix breakage in pg_upgrade's enum-type support: somebody didn't
fix it when the noise word VALUE got added to ALTER TYPE ADD.  Also,
rationalize parsetree representation of COMMENT ON DOMAIN and fix
get_object_address() to allow OBJECT_DOMAIN.

d9572c4e3b Core support for "extensions", which are packages of SQL objects.
This patch adds the server infrastructure to support extensions.
There is still one significant loose end, namely how to make it play nice
with pg_upgrade, so I am not yet committing the changes that would make
all the contrib modules depend on this feature.

In passing, fix a disturbingly large amount of breakage in
AlterObjectNamespace() and callers.

0d692a0dc9 Basic foreign table support.
Foreign tables are a core component of SQL/MED.  This commit does
not provide a working SQL/MED infrastructure, because foreign tables
cannot yet be queried.  Support for foreign table scans will need to
be added in a future patch.  However, this patch creates the necessary
system catalog structure, syntax support, and support for ancillary
operations such as COMMENT and SECURITY LABEL.

35670340f5 Refactor typenameTypeId()
Split the old typenameTypeId() into two functions: A new typenameTypeId() that
returns only a type OID, and typenameTypeIdAndMod() that returns type OID and
typmod.  This isolates call sites better that actually care about the typmod.

fd1843ff89 Standardize get_whatever_oid functions for other object types.
- Rename TSParserGetPrsid to get_ts_parser_oid.
- Rename TSDictionaryGetDictid to get_ts_dict_oid.
- Rename TSTemplateGetTmplid to get_ts_template_oid.
- Rename TSConfigGetCfgid to get_ts_config_oid.
- Rename FindConversionByName to get_conversion_oid.
- Rename GetConstraintName to get_constraint_oid.
- Add new functions get_opclass_oid, get_opfamily_oid, get_rewrite_oid,
  get_rewrite_oid_without_relid, get_trigger_oid, and get_cast_oid.

The name of each function matches the corresponding catalog.

- Add get_namespace_oid, get_language_oid, get_am_oid.
- Refactor existing code to use new interfaces.

e26c539e9f Wrap calls to SearchSysCache and related functions using macros.
The purpose of this change is to eliminate the need for every caller
of SearchSysCache, SearchSysCacheCopy, SearchSysCacheExists,
GetSysCacheOid, and SearchSysCacheList to know the maximum number
of allowable keys for a syscache entry (currently 4).  This will
make it far easier to increase the maximum number of keys in a
future release should we choose to do so, and it makes the code
shorter, too.

b7d6795445 Disallow comments on columns of relation types other than tables, views,
and composite types, which are the only relkinds for which pg_dump support
exists for dumping column comments.  There is no obvious usefulness for
comments on columns of sequences or toast tables; and while comments on
index columns might have some value, it's not worth the risk of compatibility
problems due to possible changes in the algorithm for assigning names to
index columns.  Per discussion.

In consequence, remove now-dead code for copying such comments in CREATE TABLE
LIKE.

902d1cb35f Remove all uses of the deprecated functions heap_formtuple, heap_modifytuple,
and heap_deformtuple in favor of the newer functions heap_form_tuple et al
(which do the same things but use bool control flags instead of arbitrary
char values).  Eliminate the former duplicate coding of these functions,
reducing the deprecated functions to mere wrappers around the newer ones.
We can't get rid of them entirely because add-on modules probably still
contain many instances of the old coding style.

a3540b0f65 Improve our #include situation by moving pointer types away from the
corresponding struct definitions.  This allows other headers to avoid including
certain highly-loaded headers such as rel.h and relscan.h, instead using just
relcache.h, heapam.h or genam.h, which are more lightweight and thus cause less
unnecessary dependencies.

tqual.h into heapam.h.  This makes all inclusion of tqual.h explicit.

220db7ccd8 Simplify and standardize conversions between TEXT datums and ordinary C
strings.  This patch introduces four support functions cstring_to_text,
cstring_to_text_with_len, text_to_cstring, and text_to_cstring_buffer, and
two macros CStringGetTextDatum and TextDatumGetCString.  A number of
existing macros that provided variants on these themes were removed.

Most of the places that need to make such conversions now require just one
function or macro call, in place of the multiple notational layers that used
to be needed.  There are no longer any direct calls of textout or textin,
and we got most of the places that were using handmade conversions via
memcpy (there may be a few still lurking, though).

This commit doesn't make any serious effort to eliminate transient memory
leaks caused by detoasting toasted text objects before they reach
text_to_cstring.  We changed PG_GETARG_TEXT_P to PG_GETARG_TEXT_PP in a few
places where it was easy, but much more could be done.

0bd4da23a4 Ensure that typmod decoration on a datatype name is validated in all cases,
even in code paths where we don't pay any subsequent attention to the typmod
value.  This seems needed in view of the fact that 8.3's generalized typmod
support will accept a lot of bogus syntax, such as "timestamp(foo)" or
"record(int, 42)" --- if we allow such things to pass without comment,
users will get confused.  Per a recent example from Greg Stark.

To implement this in a way that's not very vulnerable to future
bugs-of-omission, refactor the API of parse_type.c's TypeName lookup routines
so that typmod validation is folded into the base lookup operation.  Callers
can still choose not to receive the encoded typmod, but we'll check the
decoration anyway if it's present.

140d4ebcb4 Tsearch2 functionality migrates to core. The bulk of this work is by
Oleg Bartunov and Teodor Sigaev, but I did a lot of editorializing,
so anything that's broken is probably my fault.

Documentation is nonexistent as yet, but let's land the patch so we can
get some portability testing done.

8b4ff8b6a1 Wording cleanup for error messages. Also change can't -> cannot.
Standard English uses "may", "can", and "might" in different ways:

        may - permission, "You may borrow my rake."

        can - ability, "I can lift that log."

        might - possibility, "It might rain today."

Unfortunately, in conversational English, their use is often mixed, as
in, "You may use this variable to do X", when in fact, "can" is a better
choice.  Similarly, "It may crash" is better stated, "It might crash".

7bae5a289c Get rid of the separate RULE privilege for tables: now only a table's owner
can create or modify rules for the table.  Do setRuleCheckAsUser() while
loading rules into the relcache, rather than when defining a rule.  This
ensures that permission checks for tables referenced in a rule are done with
respect to the current owner of the rule's table, whereas formerly ALTER TABLE
OWNER would fail to update the permission checking for associated rules.
Removal of separate RULE privilege is needed to prevent various scenarios
in which a grantee of RULE privilege could effectively have any privilege
of the table owner.  For backwards compatibility, GRANT/REVOKE RULE is still
accepted, but it doesn't do anything.  Per discussion here:

3651a3e6fb Support the syntax
CREATE AGGREGATE aggname (input_type) (parameter_list)
along with the old syntax where the input type was named in the parameter
list.  This fits more naturally with the way that the aggregate is identified
in DROP AGGREGATE and other utility commands; furthermore it has a natural
extension to handle multiple-input aggregates, where the basetype-parameter
method would get ugly.  In fact, this commit fixes the grammar and all the
utility commands to support multiple-input aggregates; but DefineAggregate
rejects it because the executor isn't fixed yet.
I didn't do anything about treating agg(*) as a zero-input aggregate instead
of artificially making it a one-input aggregate, but that should be considered
in combination with supporting multi-input aggregates.

20ab467d76 Improve parser so that we can show an error cursor position for errors
during parse analysis, not only errors detected in the flex/bison stages.
This is per my earlier proposal.  This commit includes all the basic
infrastructure, but locations are only tracked and reported for errors
involving column references, function calls, and operators.  More could
be done later but this seems like a good set to start with.  I've also
moved the ReportSyntaxErrorPosition logic out of psql and into libpq,
which should make it available to more people --- even within psql this
is an improvement because warnings weren't handled by ReportSyntaxErrorPosition.

f9a726aa88 I've created a new shared catalog table pg_shdescription to store
comments on cluster global objects like databases, tablespaces, and
roles.

It touches a lot of places, but not much in the way of big changes.  The
only design decision I made was to duplicate the query and manipulation
functions rather than to try and have them handle both shared and local
comments.  I believe this is simpler for the code and not an issue for
callers because they know what type of object they are dealing with.
This has resulted in a shobj_description function analagous to
obj_description and backend functions [Create/Delete]SharedComments
mirroring the existing [Create/Delete]Comments functions.

pg_shdescription.h goes into src/include/catalog/

436a2956d8 Re-run pgindent, fixing a problem where comment lines after a blank
comment line where output as too long, and update typedefs for /lib
directory.  Also fix case where identifiers were used as variable names
in the backend, but as typedefs in ecpg (favor the backend for
indenting).

Backpatch to 8.1.X.

162bd08b3f Completion of project to use fixed OIDs for all system catalogs and
indexes.  Replace all heap_openr and index_openr calls by heap_open
and index_open.  Remove runtime lookups of catalog OID numbers in
various places.  Remove relcache's support for looking up system
catalogs by name.  Bulky but mostly very boring patch ...

7c13781ee7 First phase of project to use fixed OIDs for all system catalogs and
indexes.  Extend the macros in include/catalog/*.h to carry the info
about hand-assigned OIDs, and adjust the genbki script and bootstrap
code to make the relations actually get those OIDs.  Remove the small
number of RelOid_pg_foo macros that we had in favor of a complete
set named like the catname.h and indexing.h macros.  Next phase will
get rid of internal use of names for looking up catalogs and indexes;
but this completes the changes forcing an initdb, so it looks like a
good place to commit.
Along the way, I made the shared relations (pg_database etc) not be
'bootstrap' relations any more, so as to reduce the number of hardwired
entries and simplify changing those relations in future.  I'm not
sure whether they ever really needed to be handled as bootstrap
relations, but it seems to work fine to not do so now.

extend to 2005 ... first run here was very simple ... change everything
where: grep 1996-2004 && the word 'Copyright' ... scanned through the
generated list with 'less' first, and after, to make sure that I only
picked up the right entries ...
