Michael Paquier 9add405014 doc: Refresh more URLs in the docs
This updates some URLs that are redirections, mostly to an equivalent
using https.  One URL referring to generalized partial indexes was
outdated.

Peter Eisentraut 3c49c6facb Convert documentation to DocBook XML
Since some preparation work had already been done, the only source
changes left were changing empty-element tags like <xref linkend="foo">
to <xref linkend="foo"/>, and changing the DOCTYPE.

The source files are still named *.sgml, but they are actually XML files
now.  Renaming could be considered later.

In the build system, the intermediate step to convert from SGML to XML
is removed.  Everything is build straight from the source files again.
The OpenSP (or the old SP) package is no longer needed.

The documentation toolchain instructions are updated and are much
simpler now.

Peter Eisentraut c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

Tom Lane e9a3c047a5 Always use -fPIC, not -fpic, when building shared libraries with gcc.
On some platforms, -fpic fails for sufficiently large shared libraries.
We've mostly not hit that boundary yet, but there are some extensions
such as Citus and pglogical where it's becoming a problem.  A bit of
research suggests that the penalty for -fPIC is small, in the
single-digit-percentage range --- and there's none at all on popular
platforms such as x86_64.  So let's just default to -fPIC everywhere
and provide one less thing for extension developers to worry about.

Per complaint from Christoph Berg.  Back-patch to all supported branches.
(I did not bother to touch the recently-removed Makefiles for sco and
unixware in the back branches, though.  We'd have no way to test that
it doesn't break anything on those platforms.)

Tom Lane 2b860f52ed Remove "sco" and "unixware" ports.
SCO OpenServer and SCO UnixWare are more or less dead platforms.
We have never had a buildfarm member testing the "sco" port, and
the last "unixware" member was last heard from in 2012, so it's
fair to doubt that the code even compiles anymore on either one.
Remove both ports.  We can always undo this if someone shows up
with an interest in maintaining and testing these platforms.

Tom Lane da6c4f6ca8 Refer to OS X as "macOS", except for the port name which is still "darwin".
We weren't terribly consistent about whether to call Apple's OS "OS X"
or "Mac OS X", and the former is probably confusing to people who aren't
Apple users.  Now that Apple has rebranded it "macOS", follow their lead
to establish a consistent naming pattern.  Also, avoid the use of the
ancient project name "Darwin", except as the port code name which does not
seem desirable to change.  (In short, this patch touches documentation and
comments, but no actual code.)

I didn't touch contrib/start-scripts/osx/, either.  I suspect those are
obsolete and due for a rewrite, anyway.

I dithered about whether to apply this edit to old release notes, but
those were responsible for quite a lot of the inconsistencies, so I ended
up changing them too.  Anyway, Apple's being ahistorical about this,
so why shouldn't we be?

Andres Freund a6d488cb53 Remove Alpha and Tru64 support.
Support for running postgres on Alpha hasn't been tested for a long
while. Due to Alpha's uniquely lax cache coherency model it's a hard
to develop for platform (especially blindly!) and thought to be
unlikely to currently work correctly.

As Alpha is the only supported architecture for Tru64 drop support for
it as well. Tru64's support has ended 2012 and it has been in
maintenance-only mode for much longer.

Also remove stray references to __ksr__ and ultrix defines.

Peter Eisentraut 3a9d430af5 doc: Fix DocBook XML validity
The main problem is that DocBook SGML allows indexterm elements just
about everywhere, but DocBook XML is stricter.  For example, this common
pattern

    <varlistentry>
     <indexterm>...</indexterm>
     <term>...</term>
     ...
    </varlistentry>

needs to be changed to something like

    <varlistentry>
     <term>...<indexterm>...</indexterm></term>
     ...
    </varlistentry>

There is currently nothing in the build system that enforces that things
stay valid, because that requires additional tools and will receive
separate consideration.

Robert Haas ea91a6be89 Remove IRIX port.
Development of IRIX has been discontinued, and support is scheduled
to end in December of 2013.  Therefore, there will be no supported
versions of this operating system by the time PostgreSQL 9.4 is
released.  Furthermore, we have no maintainer for this platform.

Peter Eisentraut 6dcce3985b Remove unnecessary xref endterm attributes and title ids
The endterm attribute is mainly useful when the toolchain does not support
automatic link target text generation for a particular situation.  In  the
past, this was required by the man page tools for all reference page links,
but that is no longer the case, and it now actually gets in the way of
proper automatic link text generation.  The only remaining use cases are
currently xrefs to refsects.

Neil Conway 3b0453b24f Various documentation improvements. Add a lot of index entries for
the builtin functions (although some more entries are still needed),
and remove the duplicate index entries that have been causing
collateindex.pl warnings. Consistently use "int" and "bigint", rather
than a mix of "int", "integer", "int4", "bigint", and "int8". Make
parenthesis style in syntax examples more consistent. Various
copy-editing for newly-added documentation and SGML markup fixes.

Peter Eisentraut a6554df4f7 In an effort to reduce the total number of chapters, combine the small
chapters on extending types, operators, and aggregates into the extending
functions chapter.  Move the information on how to call table functions
into the queries chapter.  Remove some outdated information that is
already present in a better form in other parts of the documentation.

Bruce Momjian be2b660ecd This patch includes a lot of minor cleanups to the SGML documentation,
including:

- replacing all the appropriate usages of <citetitle>PostgreSQL
...</citetitle> with &cite-user;, &cite-admin;, and so on

- fix an omission in the EXECUTE documentation

- add some more text to the EXPLAIN documentation

- improve the PL/PgSQL RETURN NEXT documentation (more work to do here)

- minor markup fixes
