23bccc823d Fix inconsistencies and typos in the tree
This is numbered take 7, and addresses a set of issues with code
comments, variable names and unreferenced variables.

c0d0e54084 Replace references to mailinglists with @lists.postgresql.org
The namespace for all lists have changed a while ago, so all references
should use the correct address.

0e10040e19 Remove references to Majordomo
Lists are not handled by Majordomo anymore and haven't been for a while,
so remove the reference and instead direct people to the list server.

a06e56b247 doc: Update redirecting links
Update links that resulted in redirects.  Most are changes from http to
https, but there are also some other minor edits.  (There are still some
redirects where the target URL looks less elegant than the one we
currently have.  I have left those as is.)

3c49c6facb Convert documentation to DocBook XML
Since some preparation work had already been done, the only source
changes left were changing empty-element tags like <xref linkend="foo">
to <xref linkend="foo"/>, and changing the DOCTYPE.

The source files are still named *.sgml, but they are actually XML files
now.  Renaming could be considered later.

In the build system, the intermediate step to convert from SGML to XML
is removed.  Everything is build straight from the source files again.
The OpenSP (or the old SP) package is no longer needed.

The documentation toolchain instructions are updated and are much
simpler now.

c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

7f77cbd996 Change documentation references to PG website to use https: not http:
This is more secure, and saves a redirect since we no longer accept
plain HTTP connections on the website.

References in code comments should probably be updated too, but
that doesn't seem to need back-patching, whereas this does.

Also, in the 9.2 branch, remove suggestion that you can get the
source code via FTP, since that service will be shut down soon.

e228624011 Remove references to pgsql-ports and pgsql-patches mailing lists from
various documentation, since those lists are now dead/deprecated.
Point to pgsql-bugs and/or pgsql-hackers as appropriate.

a134ee3379 Update documentation on may/can/might:
Standard English uses "may", "can", and "might" in different ways:

        may - permission, "You may borrow my rake."

        can - ability, "I can lift that log."

        might - possibility, "It might rain today."

Unfortunately, in conversational English, their use is often mixed, as
in, "You may use this variable to do X", when in fact, "can" is a better
choice.  Similarly, "It may crash" is better stated, "It might crash".

Also update two error messages mentioned in the documenation to match.

5266f221a2 Merge postmaster and postgres command into just postgres. postmaster
symlink is kept for now for compatibility.  To call single-user mode, use
postgres --single.

0e2f88a6d5 We don't put URL's in ulink's because the URL is always generated, but
we can put words in ulink and the URL will still be printed.

cda1bd6cfe Various minor improvements to the SGML documentation: fix some incorrect
SGML markup, add a "deprecated features" section to the 8.0 release
notes, untabify release.sgml and runtime.sgml, and make some other
minor improvements.

80ec228389 Refer to GUC variables using <xref> tags rather than <varname> tags,
where appropriate. Add "id" and "xreflabel" tags to the descriptions
of the GUC variables to facilitate this. Also make a few minor docs
cleanups.
