0ab9c56d0f Support renaming an existing value of an enum type.
Not much to be said about this patch: it does what it says on the tin.

In passing, rename AlterEnumStmt.skipIfExists to skipIfNewValExists
to clarify what it actually does.  In the discussion of this patch
we considered supporting other similar options, such as IF EXISTS
on the type as a whole or IF NOT EXISTS on the target name.  This
patch doesn't actually add any such feature, but it might happen later.

65a588b4c3 Try to fix portability issue in enum renumbering (again).
The hack embodied in commit 4ba61a487 no longer works after today's change
to allow DatumGetFloat4/Float4GetDatum to be inlined (commit 14cca1bf8).
Probably what's happening is that the faulty compilers are deciding that
the now-inlined assignment is a no-op and so they're not required to
round to float4 width.

We had a bunch of similar issues earlier this year in the degree-based
trig functions, and eventually settled on using volatile intermediate
variables as the least ugly method of forcing recalcitrant compilers
to do what the C standard says (cf commit 82311bcdd).  Let's see if
that method works here.

30982be4e5 Integrate pg_upgrade_support module into backend
Previously, these functions were created in a schema "binary_upgrade",
which was deleted after pg_upgrade was finished.  Because we don't want
to keep that schema around permanently, move them to pg_catalog but
rename them with a binary_upgrade_... prefix.

The provided functions are only small wrappers around global variables
that were added specifically for pg_upgrade use, so keeping the module
separate does not create any modularity.

The functions still check that they are only called in binary upgrade
mode, so it is not possible to call these during normal operation.

a7ae1dcf49 pg_upgrade: prevent automatic oid assignment
Prevent automatic oid assignment when in binary upgrade mode.  Also
throw an error when contrib/pg_upgrade_support functions are called when
not in binary upgrade mode.

This prevent automatically-assigned oids from conflicting with later
pre-assigned oids coming from the old cluster.  It also makes sure oids
are preserved in call important cases.

c9e2e2db5c Partially restore comments discussing enum renumbering hazards.
As noted by Tom Lane, commit 813fb03155
was overly optimistic about how safe it is to concurrently change
enumsortorder values under MVCC catalog scan semantics.  Restore
some of the previous text, with hopefully-correct adjustments for
the new state of play.

813fb03155 Remove SnapshotNow and HeapTupleSatisfiesNow.
We now use MVCC catalog scans, and, per discussion, have eliminated
all other remaining uses of SnapshotNow, so that we can now get rid of
it.  This will break third-party code which is still using it, which
is intentional, as we want such code to be updated to do things the
new way.

568d4138c6 Use an MVCC snapshot, rather than SnapshotNow, for catalog scans.
SnapshotNow scans have the undesirable property that, in the face of
concurrent updates, the scan can fail to see either the old or the new
versions of the row.  In many cases, we work around this by requiring
DDL operations to hold AccessExclusiveLock on the object being
modified; in some cases, the existing locking is inadequate and random
failures occur as a result.  This commit doesn't change anything
related to locking, but will hopefully pave the way to allowing lock
strength reductions in the future.

The major issue has held us back from making this change in the past
is that taking an MVCC snapshot is significantly more expensive than
using a static special snapshot such as SnapshotNow.  However, testing
of various worst-case scenarios reveals that this problem is not
severe except under fairly extreme workloads.  To mitigate those
problems, we avoid retaking the MVCC snapshot for each new scan;
instead, we take a new snapshot only when invalidation messages have
been processed.  The catcache machinery already requires that
invalidation messages be sent before releasing the related heavyweight
lock; else other backends might rely on locally-cached data rather
than scanning the catalog at all.  Thus, making snapshot reuse
dependent on the same guarantees shouldn't break anything that wasn't
already subtly broken.

31510194cc Minor corrections for ALTER TYPE ADD VALUE IF NOT EXISTS patch.
Produce a NOTICE when the label already exists, for consistency with other
CREATE IF NOT EXISTS commands.  Also, fix the code so it produces something
more user-friendly than an index violation when the label already exists.
This not incidentally enables making a regression test that the previous
patch didn't make for fear of exposing an unpredictable OID in the results.
Also some wordsmithing on the documentation.

6d12b68cd7 Allow IF NOT EXISTS when add a new enum label.
If the label is already in the enum the statement becomes a no-op.
This will reduce the pain that comes from our not allowing this
operation inside a transaction block.

c219d9b0a5 Split tuple struct defs from htup.h to htup_details.h
This reduces unnecessary exposure of other headers through htup.h, which
is very widely included by many files.

I have chosen to move the function prototypes to the new file as well,
because that means htup.h no longer needs to include tupdesc.h.  In
itself this doesn't have much effect in indirect inclusion of tupdesc.h
throughout the tree, because it's also required by execnodes.h; but it's
something to explore in the future, and it seemed best to do the htup.h
change now while I'm busy with it.

fda0594fc2 remove catcache.h from syscache.h
Instead, place a forward struct declaration for struct catclist in
syscache.h.  This reduces header proliferation somewhat.

76dd09bbec Add postmaster/postgres undocumented -b option for binary upgrades.
This option turns off autovacuum, prevents non-super-user connections,
and enables oid setting hooks in the backend.  The code continues to use
the old autoavacuum disable settings for servers with earlier catalog
versions.

This includes a catalog version bump to identify servers that support
the -b option.

84c123be1d Allow new values to be added to an existing enum type.
After much expenditure of effort, we've got this to the point where the
performance penalty is pretty minimal in typical cases.

902d1cb35f Remove all uses of the deprecated functions heap_formtuple, heap_modifytuple,
and heap_deformtuple in favor of the newer functions heap_form_tuple et al
(which do the same things but use bool control flags instead of arbitrary
char values).  Eliminate the former duplicate coding of these functions,
reducing the deprecated functions to mere wrappers around the newer ones.
We can't get rid of them entirely because add-on modules probably still
contain many instances of the old coding style.

a3540b0f65 Improve our #include situation by moving pointer types away from the
corresponding struct definitions.  This allows other headers to avoid including
certain highly-loaded headers such as rel.h and relscan.h, instead using just
relcache.h, heapam.h or genam.h, which are more lightweight and thus cause less
unnecessary dependencies.

73b0300b2a Move the HTSU_Result enum definition into snapshot.h, to avoid including
tqual.h into heapam.h.  This makes all inclusion of tqual.h explicit.

I also sorted alphabetically the includes on some source files.
