9089287aa0 Guard against null plan pointer in CachedPlanIsSimplyValid().
If both the passed-in plan pointer and plansource->gplan are
NULL, CachedPlanIsSimplyValid would think that the plan pointer
is possibly-valid and try to dereference it.  For the one extant
call site in plpgsql, this situation doesn't normally happen
which is why we've not noticed. However, it appears to be possible
if the previous use of the cached plan failed, as per report from
Justin Pryzby.  Add an extra check to prevent crashing.
Back-patch to v13 where this code was added.

555b929bbe Avoid Assert failure when processing empty statement in aborted xact.
exec_parse_message() wants to create a cached plan in all cases,
including for empty input.  The empty-input path does not have
a test for being in an aborted transaction, making it possible
that plancache.c will fail due to trying to do database lookups
even though there's no real work to do.

One solution would be to throw an aborted-transaction error in
this path too, but it's not entirely clear whether the lack of
such an error was intentional or whether some clients might be
relying on non-error behavior.  Instead, let's hack plancache.c
so that it treats empty statements with the same logic it
already had for transaction control commands, ensuring that it
can soldier through even in an already-aborted transaction.

Per bug #17983 from Alexander Lakhin.  Back-patch to all
supported branches.

65eb2d00c6 Acquire locks on views in AcquirePlannerLocks, too.
Commit 47bb9db75 taught AcquireExecutorLocks to re-acquire locks
on views using data from their RTE_SUBQUERY replacements, but
it now seems like we should make AcquirePlannerLocks do the same.
In this way, if a view has been redefined, we will notice that
a bit earlier while checking validity of a cached plan and thereby
avoid some wasted work.

47bb9db759 Get rid of the "new" and "old" entries in a view's rangetable.
The rule system needs "old" and/or "new" pseudo-RTEs in rule actions
that are ON INSERT/UPDATE/DELETE.  Historically it's put such entries
into the ON SELECT rules of views as well, but those are really quite
vestigial.  The only thing we've used them for is to carry the
view's relid forward to AcquireExecutorLocks (so that we can
re-lock the view to verify it hasn't changed before re-using a plan)
and to carry its relid and permissions data forward to execution-time
permissions checks.  What we can do instead of that is to retain
these fields of the RTE_RELATION RTE for the view even after we
convert it to an RTE_SUBQUERY RTE.  This requires a tiny amount of
extra complication in the planner and AcquireExecutorLocks, but on
the other hand we can get rid of the logic that moves that data from
one place to another.

The principal immediate benefit of doing this, aside from a small
saving in the pg_rewrite data for views, is that these pseudo-RTEs
no longer trigger ruleutils.c's heuristic about qualifying variable
names when the rangetable's length is more than 1.  That results
in quite a number of small simplifications in regression test outputs,
which are all to the good IMO.

Bump catversion because we need to dump a few more fields of
RTE_SUBQUERY RTEs.  While those will always be zeroes anyway in
stored rules (because we'd never populate them until query rewrite)
they are useful for debugging, and it seems like we'd better make
sure to transmit such RTEs accurately in plans sent to parallel
workers.  I don't think the executor actually examines these fields
after startup, but someday it might.

This is a second attempt at committing 1b4d280ea.  The difference
from the first time is that now we can add some filtering rules to
AdjustUpgrade.pm to allow cross-version upgrade testing to pass
despite all the cosmetic changes in CREATE VIEW outputs.

This reverts commit 1b4d280ea1.
It's broken the buildfarm members that run cross-version-upgrade tests,
because they're not prepared to deal with cosmetic differences between
CREATE VIEW commands emitted by older servers and HEAD.  Even if we had
a solution to that, which we don't, it'd take some time to roll it out
to the affected animals.  This improvement isn't valuable enough to
justify addressing that problem on an emergency basis, so revert it
for now.

d9d873bac6 Clean up some inconsistencies with GUC declarations
This is similar to 7d25958, and this commit takes care of all the
remaining inconsistencies between the initial value used in the C
variable associated to a GUC and its default value stored in the GUC
tables (as of pg_settings.boot_val).

Some of the initial values of the GUCs updated rely on a compile-time
default.  These are refactored so as the GUC table and its C declaration
use the same values.  This makes everything consistent with other
places, backend_flush_after, bgwriter_flush_after, port,
checkpoint_flush_after doing so already, for example.

Extracted from a larger patch by Peter Smith.  The spots updated in the
modules are from me.

791b1b71da Parse/analyze function renaming
There are three parallel ways to call parse/analyze: with fixed
parameters, with variable parameters, and by supplying your own parser
callback.  Some of the involved functions were confusingly named and
made this API structure more confusing.  This patch renames some
functions to make this clearer:

parse_analyze() -> parse_analyze_fixedparams()
pg_analyze_and_rewrite() -> pg_analyze_and_rewrite_fixedparams()

(Otherwise one might think this variant doesn't accept parameters, but
in fact all three ways accept parameters.)

pg_analyze_and_rewrite_params() -> pg_analyze_and_rewrite_withcb()

(Before, and also when considering pg_analyze_and_rewrite(), one might
think this is the only way to pass parameters.  Moreover, the parser
callback doesn't necessarily need to parse only parameters, it's just
one of the things it could do.)

parse_fixed_parameters() -> setup_parse_fixed_parameters()
parse_variable_parameters() -> setup_parse_variable_parameters()

(These functions don't actually do any parsing, they just set up
callbacks to use during parsing later.)

This patch also adds some const decorations to the fixed-parameters
API, so the distinction from the variable-parameters API is more
clear.

d68a003912 Rename debug_invalidate_system_caches_always to debug_discard_caches.
The name introduced by commit 4656e3d66 was agreed to be unreasonably
long.  To match this change, rename initdb's recently-added
--clobber-cache option to --discard-caches.

def5b065ff Initial pgindent and pgperltidy run for v14.
Also "make reformat-dat-files".

The only change worthy of note is that pgindent messed up the formatting
of launcher.c's struct LogicalRepWorkerId, which led me to notice that
that struct wasn't used at all anymore, so I just took it out.

26acb54a13 Revert "Enable parallel SELECT for "INSERT INTO ... SELECT ..."."
To allow inserts in parallel-mode this feature has to ensure that all the
constraints, triggers, etc. are parallel-safe for the partition hierarchy
which is costly and we need to find a better way to do that. Additionally,
we could have used existing cached information in some cases like indexes,
domains, etc. to determine the parallel-safety.

05c8482f7f Enable parallel SELECT for "INSERT INTO ... SELECT ...".
Parallel SELECT can't be utilized for INSERT in the following cases:
- INSERT statement uses the ON CONFLICT DO UPDATE clause
- Target table has a parallel-unsafe: trigger, index expression or
  predicate, column default expression or check constraint
- Target table has a parallel-unsafe domain constraint on any column
- Target table is a partitioned table with a parallel-unsafe partition key
  expression or support function

The planner is updated to perform additional parallel-safety checks for
the cases listed above, for determining whether it is safe to run INSERT
in parallel-mode with an underlying parallel SELECT. The planner will
consider using parallel SELECT for "INSERT INTO ... SELECT ...", provided
nothing unsafe is found from the additional parallel-safety checks, or
from the existing parallel-safety checks for SELECT.

While checking parallel-safety, we need to check it for all the partitions
on the table which can be costly especially when we decide not to use a
parallel plan. So, in a separate patch, we will introduce a GUC and or a
reloption to enable/disable parallelism for Insert statements.

Prior to entering parallel-mode for the execution of INSERT with parallel
SELECT, a TransactionId is acquired and assigned to the current
transaction state. This is necessary to prevent the INSERT from attempting
to assign the TransactionId whilst in parallel-mode, which is not allowed.
This approach has a disadvantage in that if the underlying SELECT does not
return any rows, then the TransactionId is not used, however that
shouldn't happen in practice in many cases.

ee895a655c Improve performance of repeated CALLs within plpgsql procedures.
This patch essentially is cleaning up technical debt left behind
by the original implementation of plpgsql procedures, particularly
commit d92bc83c4.  That patch (or more precisely, follow-on patches
fixing its worst bugs) forced us to re-plan CALL and DO statements
each time through, if we're in a non-atomic context.  That wasn't
for any fundamental reason, but just because use of a saved plan
requires having a ResourceOwner to hold a reference count for the
plan, and we had no suitable resowner at hand, nor would the
available APIs support using one if we did.  While it's not that
expensive to create a "plan" for CALL/DO, the cycles do add up
in repeated executions.

This patch therefore makes the following API changes:

* GetCachedPlan/ReleaseCachedPlan are modified to let the caller
specify which resowner to use to pin the plan, rather than forcing
use of CurrentResourceOwner.

* spi.c gains a "SPI_execute_plan_extended" entry point that lets
callers say which resowner to use to pin the plan.  This borrows the
idea of an options struct from the recently added SPI_prepare_extended,
hopefully allowing future options to be added without more API breaks.
This supersedes SPI_execute_plan_with_paramlist (which I've marked
deprecated) as well as SPI_execute_plan_with_receiver (which is new
in v14, so I just took it out altogether).

* I also took the opportunity to remove the crude hack of letting
plpgsql reach into SPI private data structures to mark SPI plans as
"no_snapshot".  It's better to treat that as an option of
SPI_prepare_extended.

Now, when running a non-atomic procedure or DO block that contains
any CALL or DO commands, plpgsql creates a ResourceOwner that
will be used to pin the plans of the CALL/DO commands.  (In an
atomic context, we just use CurrentResourceOwner, as before.)
Having done this, we can just save CALL/DO plans normally,
whether or not they are used across transaction boundaries.
This seems to be good for something like 2X speedup of a CALL
of a trivial procedure with a few simple argument expressions.
By restricting the creation of an extra ResourceOwner like this,
there's essentially zero penalty in cases that can't benefit.

4656e3d668 Replace CLOBBER_CACHE_ALWAYS with run-time GUC
Forced cache invalidation (CLOBBER_CACHE_ALWAYS) has been impractical
to use for testing in PostgreSQL because it's so slow and because it's
toggled on/off only at build time.  It is helpful when hunting bugs in
any code that uses the sycache/relcache because causes cache
invalidations to be injected whenever it would be possible for an
invalidation to occur, whether or not one was really pending.

Address this by providing run-time control over cache clobber
behaviour using the new debug_invalidate_system_caches_always GUC.
Support is not compiled in at all unless assertions are enabled or
CLOBBER_CACHE_ENABLED is explicitly defined at compile time.  It
defaults to 0 if compiled in, so it has negligible effect on assert
build performance by default.

When support is compiled in, test code can now set
debug_invalidate_system_caches_always=1 locally to a backend to test
specific queries, functions, extensions, etc.  Or tests can toggle it
globally for a specific test case while retaining normal performance
during test setup and teardown.

For backwards compatibility with existing test harnesses and scripts,
debug_invalidate_system_caches_always defaults to 1 if
CLOBBER_CACHE_ALWAYS is defined, and to 3 if CLOBBER_CACHE_RECURSIVE
is defined.

CLOBBER_CACHE_ENABLED is now visible in pg_config_manual.h, as is the
related RECOVER_RELATION_BUILD_MEMORY setting for the relcache.

6aba63ef3e Allow the planner-related functions and hook to accept the query string.
This commit adds query_string argument into the planner-related functions
and hook and allows us to pass the query string to them.

Currently there is no user of the query string passed. But the upcoming patch
for the planning counters will add the planning hook function into
pg_stat_statements and the function will need the query string. So this change
will be necessary for that patch.

Also this change is useful for some extensions that want to use the query
string in their planner hook function.

fbc7a71608 Rearrange validity checks for plpgsql "simple" expressions.
Buildfarm experience shows what probably should've occurred to me before:
if a cache flush occurs partway through building a generic plan, then
the plansource may have is_valid = false even though the plan is valid.
We need to accept this case, use the generated plan, and then try to
replan the next time.  We can't try to replan immediately, because that
would produce an infinite loop in CLOBBER_CACHE_ALWAYS builds; moreover
it's really overkill.  (We can assume that the plan is valid, it's just
possibly a bit stale.  Note that the pre-existing code behaved this way,
and the non-simple-expression code paths do too.)  Conversely, not using
the generated plan would drop us into the not-a-simple-expression code
path, which is bad for performance and would also cause regression-test
failures due to visibly different error-reporting behavior.

Hence, refactor the validity-check functions so that the initial check
and recheck cases can react differently to plansource->is_valid.
This makes their usage a bit simpler, too.

8f59f6b9c0 Improve performance of "simple expressions" in PL/pgSQL.
For relatively simple expressions (say, "x + 1" or "x > 0"), plpgsql's
management overhead exceeds the cost of evaluating the expression.
This patch substantially improves that situation, providing roughly
2X speedup for such trivial expressions.

First, add infrastructure in the plancache to allow fast re-validation
of cached plans that contain no table access, and hence need no locks.
Teach plpgsql to use this infrastructure for expressions that it's
already deemed "simple" (which in particular will never contain table
references).

The fast path still requires checking that search_path hasn't changed,
so provide a fast path for OverrideSearchPathMatchesCurrent by
counting changes that have occurred to the active search path in the
current session.  This is simplistic but seems enough for now, seeing
that PushOverrideSearchPath is not used in any performance-critical
cases.

Second, manage the refcounts on simple expressions' cached plans using
a transaction-lifespan resource owner, so that we only need to take
and release an expression's refcount once per transaction not once per
expression evaluation.  The management of this resource owner exactly
parallels the existing management of plpgsql's simple-expression EState.

Add some regression tests covering this area, in particular verifying
that expression caching doesn't break semantics for search_path changes.

Patch by me, but it owes something to previous work by Amit Langote,
who recognized that getting rid of plancache-related overhead would
be a useful thing to do here.  Also thanks to Andres Freund for review.

Represent command completion tags as structs
The backend was using strings to represent command tags and doing string
comparisons in multiple places, but that's slow and unhelpful.  Create a
new command list with a supporting structure to use instead; this is
stored in a tag-list-file that can be tailored to specific purposes with
a caller-definable C macro, similar to what we do for WAL resource
managers.  The first first such uses are a new CommandTag enum and a
CommandTagBehavior struct.

Replace numerous occurrences of char *completionTag with a
QueryCompletion struct so that the code no longer stores information
about completed queries in a cstring.  Only at the last moment, in
EndCommand(), does this get converted to a string.

EventTriggerCacheItem no longer holds an array of palloc’d tag strings
in sorted order, but rather just a Bitmapset over the CommandTags.

8255c7a5ee Phase 2 pgindent run for v12.
Switch to 2.1 version of pg_bsd_indent.  This formats
multiline function declarations "correctly", that is with
additional lines of parameter declarations indented to match
where the first line's left parenthesis is.

f09346a9c6 Refactor planner's header files.
Create a new header optimizer/optimizer.h, which exposes just the
planner functions that can be used "at arm's length", without need
to access Paths or the other planner-internal data structures defined
in nodes/relation.h.  This is intended to provide the whole planner
API seen by most of the rest of the system; although FDWs still need
to use additional stuff, and more thought is also needed about just
what selfuncs.c should rely on.

The main point of doing this now is to limit the amount of new
#include baggage that will be needed by "planner support functions",
which I expect to introduce later, and which will be in relevant
datatype modules rather than anywhere near the planner.

This commit just moves relevant declarations into optimizer.h from
other header files (a couple of which go away because everything
got moved), and adjusts #include lists to match.  There's further
cleanup that could be done if we want to decide that some stuff
being exposed by optimizer.h doesn't belong in the planner at all,
but I'll leave that for another day.

04fe805a17 Drop no-op CoerceToDomain nodes from expressions at planning time.
If a domain has no constraints, then CoerceToDomain doesn't really do
anything and can be simplified to a RelabelType.  This not only
eliminates cycles at execution, but allows the planner to optimize better
(for instance, match the coerced expression to an index on the underlying
column).  However, we do have to support invalidating the plan later if
a constraint gets added to the domain.  That's comparable to the case of
a change to a SQL function that had been inlined into a plan, so all the
necessary logic already exists for plans depending on functions.  We
need only duplicate or share that logic for domains.

ALTER DOMAIN ADD/DROP CONSTRAINT need to be taught to send out sinval
messages for the domain's pg_type entry, since those operations don't
update that row.  (ALTER DOMAIN SET/DROP NOT NULL do update that row,
so no code change is needed for them.)

Testing this revealed what's really a pre-existing bug in plpgsql:
it caches the SQL-expression-tree expansion of type coercions and
had no provision for invalidating entries in that cache.  Up to now
that was only a problem if such an expression had inlined a SQL
function that got changed, which is unlikely though not impossible.
But failing to track changes of domain constraints breaks an existing
regression test case and would likely cause practical problems too.

We could fix that locally in plpgsql, but what seems like a better
idea is to build some generic infrastructure in plancache.c to store
standalone expressions and track invalidation events for them.
(It's tempting to wonder whether plpgsql's "simple expression" stuff
could use this code with lower overhead than its current use of the
heavyweight plancache APIs.  But I've left that idea for later.)

Other stuff fixed in passing:

* Allow estimate_expression_value() to drop CoerceToDomain
unconditionally, effectively assuming that the coercion will succeed.
This will improve planner selectivity estimates for cases involving
estimatable expressions that are coerced to domains.  We could have
done this independently of everything else here, but there wasn't
previously any need for eval_const_expressions_mutator to know about
CoerceToDomain at all.

* Use a dlist for plancache.c's list of cached plans, rather than a
manually threaded singly-linked list.  That eliminates a potential
performance problem in DropCachedPlan.

* Fix a couple of inconsistencies in typecmds.c about whether
operations on domains drop RowExclusiveLock on pg_type.  Our common
practice is that DDL operations do drop catalog locks, so standardize
on that choice.

578b229718 Remove WITH OIDS support, change oid catalog column visibility.
Previously tables declared WITH OIDS, including a significant fraction
of the catalog tables, stored the oid column not as a normal column,
but as part of the tuple header.

This special column was not shown by default, which was somewhat odd,
as it's often (consider e.g. pg_class.oid) one of the more important
parts of a row.  Neither pg_dump nor COPY included the contents of the
oid column by default.

The fact that the oid column was not an ordinary column necessitated a
significant amount of special case code to support oid columns. That
already was painful for the existing, but upcoming work aiming to make
table storage pluggable, would have required expanding and duplicating
that "specialness" significantly.

WITH OIDS has been deprecated since 2005 (commit ff02d0a05280e0).
Remove it.

Removing includes:
- CREATE TABLE and ALTER TABLE syntax for declaring the table to be
  WITH OIDS has been removed (WITH (oids[ = true]) will error out)
- pg_dump does not support dumping tables declared WITH OIDS and will
  issue a warning when dumping one (and ignore the oid column).
- restoring an pg_dump archive with pg_restore will warn when
  restoring a table with oid contents (and ignore the oid column)
- COPY will refuse to load binary dump that includes oids.
- pg_upgrade will error out when encountering tables declared WITH
  OIDS, they have to be altered to remove the oid column first.
- Functionality to access the oid of the last inserted row (like
  plpgsql's RESULT_OID, spi's SPI_lastoid, ...) has been removed.

The syntax for declaring a table WITHOUT OIDS (or WITH (oids = false)
for CREATE TABLE) is still supported. While that requires a bit of
support code, it seems unnecessary to break applications / dumps that
do not use oids, and are explicit about not using them.

The biggest user of WITH OID columns was postgres' catalog. This
commit changes all 'magic' oid columns to be columns that are normally
declared and stored. To reduce unnecessary query breakage all the
newly added columns are still named 'oid', even if a table's column
naming scheme would indicate 'reloid' or such.  This obviously
requires adapting a lot code, mostly replacing oid access via
HeapTupleGetOid() with access to the underlying Form_pg_*->oid column.

The bootstrap process now assigns oids for all oid columns in
genbki.pl that do not have an explicit value (starting at the largest
oid previously used), only oids assigned later by oids will be above
FirstBootstrapObjectId. As the oid column now is a normal column the
special bootstrap syntax for oids has been removed.

Oids are not automatically assigned during insertion anymore, all
backend code explicitly assigns oids with GetNewOidWithIndex(). For
the rare case that insertions into the catalog via SQL are called for
the new pg_nextoid() function can be used (which only works on catalog
tables).

The fact that oid columns on system tables are now normal columns
means that they will be included in the set of columns expanded
by * (i.e. SELECT * FROM pg_class will now include the table's oid,
previously it did not). It'd not technically be hard to hide oid
column by default, but that'd mean confusing behavior would either
have to be carried forward forever, or it'd cause breakage down the
line.

While it's not unlikely that further adjustments are needed, the
scope/invasiveness of the patch makes it worthwhile to get merge this
now. It's painful to maintain externally, too complicated to commit
after the code code freeze, and a dependency of a number of other
patches.

6e35939feb Change rewriter/planner/executor/plancache to depend on RTE rellockmode.
Instead of recomputing the required lock levels in all these places,
just use what commit fdba460a2 made the parser store in the RTE fields.
This already simplifies the code measurably in these places, and
follow-on changes will remove a bunch of no-longer-needed infrastructure.

In a few cases, this change causes us to acquire a higher lock level
than we did before.  This is OK primarily because said higher lock level
should've been acquired already at query parse time; thus, we're saving
a useless extra trip through the shared lock manager to acquire a lesser
lock alongside the original lock.  The only known exception to this is
that re-execution of a previously planned SELECT FOR UPDATE/SHARE query,
for a table that uses ROW_MARK_REFERENCE or ROW_MARK_COPY methods, might
have gotten only AccessShareLock before.  Now it will get RowShareLock
like the first execution did, which seems fine.

While there's more to do, push it in this state anyway, to let the
buildfarm help verify that nothing bad happened.

fdba460a26 Create an RTE field to record the query's lock mode for each relation.
Add RangeTblEntry.rellockmode, which records the appropriate lock mode for
each RTE_RELATION rangetable entry (either AccessShareLock, RowShareLock,
or RowExclusiveLock depending on the RTE's role in the query).

This patch creates the field and makes all creators of RTE nodes fill it
in reasonably, but for the moment nothing much is done with it.  The plan
is to replace assorted post-parser logic that re-determines the right
lockmode to use with simple uses of rte->rellockmode.  For now, just add
Asserts in each of those places that the rellockmode matches what they are
computing today.  (In some cases the match isn't perfect, so the Asserts
are weaker than you might expect; but this seems OK, as per discussion.)

This passes check-world for me, but it seems worth pushing in this state
to see if the buildfarm finds any problems in cases I failed to test.

442accc3fe Allow memory contexts to have both fixed and variable ident strings.
Originally, we treated memory context names as potentially variable in
all cases, and therefore always copied them into the context header.
Commit 9fa6f00b1 rethought this a little bit and invented a distinction
between fixed and variable names, skipping the copy step for the former.
But we can make things both simpler and more useful by instead allowing
there to be two parts to a context's identification, a fixed "name" and
an optional, variable "ident".  The name supplied in the context create
call is now required to be a compile-time-constant string in all cases,
as it is never copied but just pointed to.  The "ident" string, if
wanted, is supplied later.  This is needed because typically we want
the ident to be stored inside the context so that it's cleaned up
automatically on context deletion; that means it has to be copied into
the context before we can set the pointer.

The cost of this approach is basically just an additional pointer field
in struct MemoryContextData, which isn't much overhead, and is bought
back entirely in the AllocSet case by not needing a headerSize field
anymore, since we no longer have to cope with variable header length.
In addition, we can simplify the internal interfaces for memory context
creation still further, saving a few cycles there.  And it's no longer
true that a custom identifier disqualifies a context from participating
in aset.c's freelist scheme, so possibly there's some win on that end.

All the places that were using non-compile-time-constant context names
are adjusted to put the variable info into the "ident" instead.  This
allows more effective identification of those contexts in many cases;
for example, subsidary contexts of relcache entries are now identified
by both type (e.g. "index info") and relname, where before you got only
one or the other.  Contexts associated with PL function cache entries
are now identified more fully and uniformly, too.

I also arranged for plancache contexts to use the query source string
as their identifier.  This is basically free for CachedPlanSources, as
they contained a copy of that string already.  We pay an extra pstrdup
to do it for CachedPlans.  That could perhaps be avoided, but it would
make things more fragile (since the CachedPlanSource is sometimes
destroyed first).  I suspect future improvements in error reporting will
require CachedPlans to have a copy of that string anyway, so it's not
clear that it's worth moving mountains to avoid it now.

This also changes the APIs for context statistics routines so that the
context-specific routines no longer assume that output goes straight
to stderr, nor do they know all details of the output format.  This
is useful immediately to reduce code duplication, and it also allows
for external code to do something with stats output that's different
from printing to stderr.

The reason for pushing this now rather than waiting for v12 is that
it rethinks some of the API changes made by commit 9fa6f00b1.  Seems
better for extension authors to endure just one round of API changes
not two.

2eb4a831e5 Change TRUE/FALSE to true/false
The lower case spellings are C and C++ standard and are used in most
parts of the PostgreSQL sources.  The upper case spellings are only used
in some files/modules.  So standardize on the standard spellings.

The APIs for ICU, Perl, and Windows define their own TRUE and FALSE, so
those are left as is when using those APIs.

In code comments, we use the lower-case spelling for the C concepts and
keep the upper-case spelling for the SQL concepts.

382ceffdf7 Phase 3 of pgindent updates.
Don't move parenthesized lines to the left, even if that means they
flow past the right margin.

By default, BSD indent lines up statement continuation lines that are
within parentheses so that they start just to the right of the preceding
left parenthesis.  However, traditionally, if that resulted in the
continuation line extending to the right of the desired right margin,
then indent would push it left just far enough to not overrun the margin,
if it could do so without making the continuation line start to the left of
the current statement indent.  That makes for a weird mix of indentations
unless one has been completely rigid about never violating the 80-column
limit.

This behavior has been pretty universally panned by Postgres developers.
Hence, disable it with indent's new -lpl switch, so that parenthesized
lines are always lined up with the preceding left paren.

This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

c7b8998ebb Phase 2 of pgindent updates.
Change pg_bsd_indent to follow upstream rules for placement of comments
to the right of code, and remove pgindent hack that caused comments
following #endif to not obey the general rule.

Commit e3860ffa4d wasn't actually using
the published version of pg_bsd_indent, but a hacked-up version that
tried to minimize the amount of movement of comments to the right of
code.  The situation of interest is where such a comment has to be
moved to the right of its default placement at column 33 because there's
code there.  BSD indent has always moved right in units of tab stops
in such cases --- but in the previous incarnation, indent was working
in 8-space tab stops, while now it knows we use 4-space tabs.  So the
net result is that in about half the cases, such comments are placed
one tab stop left of before.  This is better all around: it leaves
more room on the line for comment text, and it means that in such
cases the comment uniformly starts at the next 4-space tab stop after
the code, rather than sometimes one and sometimes two tabs after.

Also, ensure that comments following #endif are indented the same
as comments following other preprocessor commands such as #else.
That inconsistency turns out to have been self-inflicted damage
from a poorly-thought-through post-indent "fixup" in pgindent.

This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

8f0530f580 Improve castNode notation by introducing list-extraction-specific variants.
This extends the castNode() notation introduced by commit 5bcab1114 to
provide, in one step, extraction of a list cell's pointer and coercion to
a concrete node type.  For example, "lfirst_node(Foo, lc)" is the same
as "castNode(Foo, lfirst(lc))".  Almost half of the uses of castNode
that have appeared so far include a list extraction call, so this is
pretty widely useful, and it saves a few more keystrokes compared to the
old way.

As with the previous patch, back-patch the addition of these macros to
pg_list.h, so that the notation will be available when back-patching.

Patch by me, after an idea of Andrew Gierth's.

41bd155dd6 Fix two undocumented parameters to functions from ENR patch.
On ProcessUtility document the parameter, to match others.

On CreateCachedPlan drop the queryEnv parameter.  It was not
referenced within the function, and had been added on the
assumption that with some unknown future usage of QueryEnvironment
it might be useful to do something there.  We have avoided other
"just in case" implementation of unused paramters, so drop it here.

18ce3a4ab2 Add infrastructure to support EphemeralNamedRelation references.
A QueryEnvironment concept is added, which allows new types of
objects to be passed into queries from parsing on through
execution.  At this point, the only thing implemented is a
collection of EphemeralNamedRelation objects -- relations which
can be referenced by name in queries, but do not exist in the
catalogs.  The only type of ENR implemented is NamedTuplestore, but
provision is made to add more types fairly easily.

An ENR can carry its own TupleDesc or reference a relation in the
catalogs by relid.

Although these features can be used without SPI, convenience
functions are added to SPI so that ENRs can easily be used by code
run through SPI.

The initial use of all this is going to be transition tables in
AFTER triggers, but that will be added to each PL as a separate
commit.

An incidental effect of this patch is to produce a more informative
error message if an attempt is made to modify the contents of a CTE
from a referencing DML statement.  No tests previously covered that
possibility, so one is added.

4cb824699e Cast result of copyObject() to correct type
copyObject() is declared to return void *, which allows easily assigning
the result independent of the input, but it loses all type checking.

If the compiler supports typeof or something similar, cast the result to
the input type.  This creates a greater amount of type safety.  In some
cases, where the result is assigned to a generic type such as Node * or
Expr *, new casts are now necessary, but in general casts are now
unnecessary in the normal case and indicate that something unusual is
happening.

d3cc37f1d8 Don't scan partitioned tables.
Partitioned tables do not contain any data; only their unpartitioned
descendents need to be scanned.  However, the partitioned tables still
need to be locked, even though they're not scanned.  To make that
work, Append and MergeAppend relations now need to carry a list of
(unscanned) partitioned relations that must be locked, and InitPlan
must lock all partitioned result relations.

Aside from the obvious advantage of avoiding some work at execution
time, this has two other advantages.  First, it may improve the
planner's decision-making in some cases since the empty relation
might throw things off.  Second, it paves the way to getting rid of
the storage for partitioned tables altogether.

7afd56c3c6 Use castNode() in a bunch of statement-list-related code.
When I wrote commit ab1f0c822, I really missed the castNode() macro that
Peter E. had proposed shortly before.  This back-fills the uses I would
have put it to.  It's probably not all that significant, but there are
more assertions here than there were before, and conceivably they will
help catch any bugs associated with those representation changes.

I left behind a number of usages like "(Query *) copyObject(query_var)".
Those could have been converted as well, but Peter has proposed another
notational improvement that would handle copyObject cases automatically,
so I let that be for now.

ab1f0c8225 Change representation of statement lists, and add statement location info.
This patch makes several changes that improve the consistency of
representation of lists of statements.  It's always been the case
that the output of parse analysis is a list of Query nodes, whatever
the types of the individual statements in the list.  This patch brings
similar consistency to the outputs of raw parsing and planning steps:

* The output of raw parsing is now always a list of RawStmt nodes;
the statement-type-dependent nodes are one level down from that.

* The output of pg_plan_queries() is now always a list of PlannedStmt
nodes, even for utility statements.  In the case of a utility statement,
"planning" just consists of wrapping a CMD_UTILITY PlannedStmt around
the utility node.  This list representation is now used in Portal and
CachedPlan plan lists, replacing the former convention of intermixing
PlannedStmts with bare utility-statement nodes.

Now, every list of statements has a consistent head-node type depending
on how far along it is in processing.  This allows changing many places
that formerly used generic "Node *" pointers to use a more specific
pointer type, thus reducing the number of IsA() tests and casts needed,
as well as improving code clarity.

Also, the post-parse-analysis representation of DECLARE CURSOR is changed
so that it looks more like EXPLAIN, PREPARE, etc.  That is, the contained
SELECT remains a child of the DeclareCursorStmt rather than getting flipped
around to be the other way.  It's now true for both Query and PlannedStmt
that utilityStmt is non-null if and only if commandType is CMD_UTILITY.
That allows simplifying a lot of places that were testing both fields.
(I think some of those were just defensive programming, but in many places,
it was actually necessary to avoid confusing DECLARE CURSOR with SELECT.)

Because PlannedStmt carries a canSetTag field, we're also able to get rid
of some ad-hoc rules about how to reconstruct canSetTag for a bare utility
statement; specifically, the assumption that a utility is canSetTag if and
only if it's the only one in its list.  While I see no near-term need for
relaxing that restriction, it's nice to get rid of the ad-hocery.

The API of ProcessUtility() is changed so that what it's passed is the
wrapper PlannedStmt not just the bare utility statement.  This will affect
all users of ProcessUtility_hook, but the changes are pretty trivial; see
the affected contrib modules for examples of the minimum change needed.
(Most compilers should give pointer-type-mismatch warnings for uncorrected
code.)

There's also a change in the API of ExplainOneQuery_hook, to pass through
cursorOptions instead of expecting hook functions to know what to pick.
This is needed because of the DECLARE CURSOR changes, but really should
have been done in 9.6; it's unlikely that any extant hook functions
know about using CURSOR_OPT_PARALLEL_OK.

Finally, teach gram.y to save statement boundary locations in RawStmt
nodes, and pass those through to Query and PlannedStmt nodes.  This allows
more intelligent handling of cases where a source query string contains
multiple statements.  This patch doesn't actually do anything with the
information, but a follow-on patch will.  (Passing this information through
cleanly is the true motivation for these changes; while I think this is all
good cleanup, it's unlikely we'd have bothered without this end goal.)

This patch is by me, but it owes a good deal to Fabien Coelho who did
a lot of preliminary work on the problem, and also reviewed the patch.

c52d37c8b3 Invalidate cached plans on FDW option changes.
This fixes problems where a plan must change but fails to do so,
as seen in a bug report from Rajkumar Raghuwanshi.

For ALTER FOREIGN TABLE OPTIONS, do this through the standard method of
forcing a relcache flush on the table.  For ALTER FOREIGN DATA WRAPPER
and ALTER SERVER, just flush the whole plan cache on any change in
pg_foreign_data_wrapper or pg_foreign_server.  That matches the way
we handle some other low-probability cases such as opclass changes, and
it's unclear that the case arises often enough to be worth working harder.
Besides, that gives a patch that is simple enough to back-patch with
confidence.

Back-patch to 9.3.  In principle we could apply the code change to 9.2 as
well, but (a) we lack postgres_fdw to test it with, (b) it's doubtful that
anyone is doing anything exciting enough with FDWs that far back to need
this desperately, and (c) the patch doesn't apply cleanly.
