An upcoming HEAD-only patch will standardize the terminology around
ItemIdData variables/line pointers, ending the practice of referring to
them as "item pointers". Make the "Database Page Layout" docs
consistent with the new policy. The term "item identifier" is already
used in the same section, so stick with that.

This feature was using a process local map to track the first few blocks
in the relation. The map was reset each time we get the block with enough
freespace. It was discussed that it would be better to track this map on
a per-relation basis in relcache and then invalidate the same whenever
vacuum frees up some space in the page or when FSM is created. The new
design would be better both in terms of API design and performance.

This adds documentation about the user oriented parts of table access
methods (i.e. the default_table_access_method GUC and the USING clause
for CREATE TABLE etc), adds a basic chapter about the table access
method interface, and adds a note to storage.sgml that it's contents
don't necessarily apply for non-builtin AMs.

refrain from creating the FSM for heaps with 4 pages or fewer. If the last
known target block has insufficient space, we still try to insert into some
other page before giving up and extending the relation, since doing
otherwise leads to table bloat. Testing showed that trying every page
penalized performance slightly, so we compromise and try every other page.
This way, we visit at most two pages. Any pages with wasted free space
become visible at next relation extension, so we still control table bloat.
As a bonus, directly attempting one or two pages can even be faster than
consulting the FSM would have been.
Once the FSM is created for a heap we don't remove it even if somebody
deletes all the rows from the corresponding relation. We don't think it is
a useful optimization as it is quite likely that relation will again grow
to the same size.

An xref to a <para>'s ID isn't very helpful because paras don't have
names. This causes a warning while building PDFs, though for some
reason not while building HTML. The link arguably went to the wrong
place, too.
To fix, introduce a sub-section we can reference.
As promised in earlier commits, this adds documentation about the new
build options, the new GUCs, about the planner logic when JIT is used,
and the benefits of JIT in general.
Also adds a more implementation oriented README.
I'm sure we're going to want to expand this further, but I think this
is a reasonable start.

Since some preparation work had already been done, the only source
changes left were changing empty-element tags like <xref linkend="foo">
to <xref linkend="foo"/>, and changing the DOCTYPE.
The source files are still named *.sgml, but they are actually XML files
now. Renaming could be considered later.
In the build system, the intermediate step to convert from SGML to XML
is removed. Everything is build straight from the source files again.
The OpenSP (or the old SP) package is no longer needed.
The documentation toolchain instructions are updated and are much
simpler now.

The syslogger will write out the current stderr and csvlog names, if
it's running and there are any, to a new file in the data directory
called "current_logfiles". We take care to remove this file when it
might no longer be valid (but not at shutdown). The function
pg_current_logfile() can be used to read the entries in the file.

"xlog" is not a particularly clear abbreviation for "write-ahead log",
and it sometimes confuses users into believe that the contents of the
"pg_xlog" directory are not critical data, leading to unpleasant
consequences. So, rename the directory to "pg_wal".
This patch modifies pg_upgrade and pg_basebackup to understand both
the old and new directory layouts; the former is necessary given the
purpose of the tool, while the latter merely avoids an unnecessary
backward-compatibility break.
We may wish to consider renaming other programs, switches, and
functions which still use the old "xlog" naming to also refer to
"wal". However, that's still under discussion, so let's do just this
much for now.

We didn't have any real user documentation about how index-only scans
work or how to design indexes to exploit them. Remedy that.

This lets you examine the visibility map as well as page-level
visibility information. I initially wrote it as a debugging aid,
but was encouraged to polish it for commit.

The new bit indicates whether every tuple on the page is already frozen.
It is cleared only when the all-visible bit is cleared, and it can be
set only when we vacuum a page and find that every tuple on that page is
both visible to every transaction and in no need of any future
vacuuming.
A future commit will use this new bit to optimize away full-table scans
that would otherwise be triggered by XID wraparound considerations. A
page which is merely all-visible must still be scanned in that case, but
a page which is all-frozen need not be. This commit does not attempt
that optimization, although that optimization is the goal here. It
seems better to get the basic infrastructure in place first.
Per discussion, it's very desirable for pg_upgrade to automatically
migrate existing VM forks from the old format to the new format. That,
too, will be handled in a follow-on patch.

This patch introduces the ability for complex datatypes to have an
in-memory representation that is different from their on-disk format.
On-disk formats are typically optimized for minimal size, and in any case
they can't contain pointers, so they are often not well-suited for
computation. Now a datatype can invent an "expanded" in-memory format
that is better suited for its operations, and then pass that around among
the C functions that operate on the datatype. There are also provisions
(rudimentary as yet) to allow an expanded object to be modified in-place
under suitable conditions, so that operations like assignment to an element
of an array need not involve copying the entire array.
The initial application for this feature is arrays, but it is not hard
to foresee using it for other container types like JSON, XML and hstore.
I have hopes that it will be useful to PostGIS as well.
In this initial implementation, a few heuristics have been hard-wired
into plpgsql to improve performance for arrays that are stored in
plpgsql variables. We would like to generalize those hacks so that
other datatypes can obtain similar improvements, but figuring out some
appropriate APIs is left as a task for future work. (The heuristics
themselves are probably not optimal yet, either, as they sometimes
force expansion of arrays that would be better left alone.)
Preliminary performance testing shows impressive speed gains for plpgsql
functions that do element-by-element access or update of large arrays.
There are other cases that get a little slower, as a result of added array
format conversions; but we can hope to improve anything that's annoyingly
bad. In any case most applications should see a net win.

While working on documentation for expanded arrays, I noticed a number of
details in the TOAST-related documentation that were already inaccurate or
obsolete. This should be fixed independently of whether expanded arrays
get in or not. One issue is that the already existing indirect-pointer
facility was not documented at all. Also, the documentation says that you
only need to use VARSIZE/SET_VARSIZE if you've made your variable-length
type TOAST-aware, but actually we've forced that business on all varlena
types even if they've opted out of TOAST by setting storage = plain.
Wordsmith a few other things too, like an amusingly archaic claim that
there are few 64-bit machines.
I thought about back-patching this, but since all this doco is oriented
to hackers and C-coded extension authors, fixing it in HEAD is probably
good enough.
Transactions can now set their commit timestamp directly as they commit,
or an external transaction commit timestamp can be fed from an outside
system using the new function TransactionTreeSetCommitTsData(). This
data is crash-safe, and truncated at Xid freeze point, same as pg_clog.
This module is disabled by default because it causes a performance hit,
but can be enabled in postgresql.conf requiring only a server restart.
A new test in src/test/modules is included.
Catalog version bumped due to the new subdirectory within PGDATA and a
couple of new SQL functions.

The initialization fork was added in 9.1, but has not been taken into
consideration in documents of get_raw_page function in pageinspect and
storage layout. This commit fixes those oversights.
get_raw_page can read not only a table but also an index, etc. So it
should be documented that the function can read any relation. This commit
also fixes the document of pageinspect that way.
Back-patch to 9.1 where those oversights existed.

The old name wasn't very descriptive as of actual contents of the
directory, which are historical snapshots in the snapshots/
subdirectory and mappingdata for rewritten tuples in
mappings/. There's been a fair amount of discussion what would be a
good name. I'm settling for pg_logical because it's likely that
further data around logical decoding and replication will need saving
in the future.
Also add the missing entry for the directory into storage.sgml's list
of PGDATA contents.
Bumps catversion as the data directories won't be compatible.
Document that routine vacuuming is now also important for the purpose
of index-only scans; and mention in the section that describes the
visibility map that it is used to implement index-only scans.

A transaction can export a snapshot with pg_export_snapshot(), and then
others can import it with SET TRANSACTION SNAPSHOT. The data does not
leave the server so there are not security issues. A snapshot can only
be imported while the exporting transaction is still running, and there
are some other restrictions.
I'm not totally convinced that we've covered all the bases for SSI (true
serializable) mode, but it works fine for lesser isolation modes.
