Noah Misch 3cd9c3b921 Fix CREATE INDEX CONCURRENTLY for the newest prepared transactions.
The purpose of commit 8a54e12a38 was to
fix this, and it sufficed when the PREPARE TRANSACTION completed before
the CIC looked for lock conflicts.  Otherwise, things still broke.  As
before, in a cluster having used CIC while having enabled prepared
transactions, queries that use the resulting index can silently fail to
find rows.  It may be necessary to reindex to recover from past
occurrences; REINDEX CONCURRENTLY suffices.  Fix this for future index
builds by making CIC wait for arbitrarily-recent prepared transactions
and for ordinary transactions that may yet PREPARE TRANSACTION.  As part
of that, have PREPARE TRANSACTION transfer locks to its dummy PGPROC
before it calls ProcArrayClearTransaction().  Back-patch to 9.6 (all
supported versions).

Amit Kapila a8fd13cab0 Add support for prepared transactions to built-in logical replication.
To add support for streaming transactions at prepare time into the
built-in logical replication, we need to do the following things:

* Modify the output plugin (pgoutput) to implement the new two-phase API
callbacks, by leveraging the extended replication protocol.

* Modify the replication apply worker, to properly handle two-phase
transactions by replaying them on prepare.

* Add a new SUBSCRIPTION option "two_phase" to allow users to enable
two-phase transactions. We enable the two_phase once the initial data sync
is over.

We however must explicitly disable replication of two-phase transactions
during replication slot creation, even if the plugin supports it. We
don't need to replicate the changes accumulated during this phase,
and moreover, we don't have a replication connection open so we don't know
where to send the data anyway.

The streaming option is not allowed with this new two_phase option. This
can be done as a separate patch.

We don't allow to toggle two_phase option of a subscription because it can
lead to an inconsistent replica. For the same reason, we don't allow to
refresh the publication once the two_phase is enabled for a subscription
unless copy_data option is false.

Fujii Masao 7b8a899bde Make pg_waldump report more detail information about PREPARE TRANSACTION record.
This commit changes xact_desc() so that it reports the detail information about
PREPARE TRANSACTION record, like GID (global transaction identifier),
timestamp at prepare transaction, delete-on-abort/commit relations,
XID of subtransactions, and invalidation messages. These are helpful
when diagnosing 2PC-related troubles.

Tom Lane 8255c7a5ee Phase 2 pgindent run for v12.
Switch to 2.1 version of pg_bsd_indent.  This formats
multiline function declarations "correctly", that is with
additional lines of parameter declarations indented to match
where the first line's left parenthesis is.

Michael Paquier effe7d9552 Make release of 2PC identifier and locks consistent in COMMIT PREPARED
When preparing a transaction in two-phase commit, a dummy PGPROC entry
holding the GID used for the transaction is registered, which gets
released once COMMIT PREPARED is run.  Prior releasing its shared memory
state, all the locks taken in the prepared transaction are released
using a dedicated set of callbacks (pgstat and multixact having similar
callbacks), which may cause the locks to be released before the GID is
set free.

Hence, there is a small window where lock conflicts could happen, for
example:
- Transaction A releases its locks, still holding its GID in shared
memory.
- Transaction B held a lock which conflicted with locks of transaction
A.
- Transaction B continues its processing, reusing the same GID as
transaction A.
- Transaction B fails because of a conflicting GID, already in use by
transaction A.

This commit changes the shared memory state release so as post-commit
callbacks and predicate lock cleanup happen consistently with the shared
memory state cleanup for the dummy PGPROC entry.  The race window is
small and 2PC had this issue from the start, so no backpatch is done.
On top if that fixes discussed involved ABI breakages, which are not
welcome in stable branches.

Simon Riggs 1eb6d6527a Store 2PC GID in commit/abort WAL recs for logical decoding
Store GID of 2PC in commit/abort WAL records when wal_level = logical.
This allows logical decoding to send the SAME gid to subscribers
across restarts of logical replication.

Track relica origin replay progress for 2PC.

Tom Lane c7b8998ebb Phase 2 of pgindent updates.
Change pg_bsd_indent to follow upstream rules for placement of comments
to the right of code, and remove pgindent hack that caused comments
following #endif to not obey the general rule.

Commit e3860ffa4d wasn't actually using
the published version of pg_bsd_indent, but a hacked-up version that
tried to minimize the amount of movement of comments to the right of
code.  The situation of interest is where such a comment has to be
moved to the right of its default placement at column 33 because there's
code there.  BSD indent has always moved right in units of tab stops
in such cases --- but in the previous incarnation, indent was working
in 8-space tab stops, while now it knows we use 4-space tabs.  So the
net result is that in about half the cases, such comments are placed
one tab stop left of before.  This is better all around: it leaves
more room on the line for comment text, and it means that in such
cases the comment uniformly starts at the next 4-space tab stop after
the code, rather than sometimes one and sometimes two tabs after.

Also, ensure that comments following #endif are indented the same
as comments following other preprocessor commands such as #else.
That inconsistency turns out to have been self-inflicted damage
from a poorly-thought-through post-indent "fixup" in pgindent.

This patch is much less interesting than the first round of indent
changes, but also bulkier, so I thought it best to separate the effects.

Simon Riggs 49e9281549 Rework handling of subtransactions in 2PC recovery
The bug fixed by 0874d4f3e1
caused us to question and rework the handling of
subtransactions in 2PC during and at end of recovery.
Patch adds checks and tests to ensure no further bugs.

This effectively removes the temporary measure put in place
by 546c13e11b.

Simon Riggs 728bd991c3 Speedup 2PC recovery by skipping two phase state files in normal path
2PC state info held in shmem at PREPARE, then cleaned at COMMIT PREPARED/ABORT PREPARED,
avoiding writing/fsyncing any state information to disk in the normal path, greatly enhancing replay speed.
Prepared transactions that live past one checkpoint redo horizon will be written to disk as now.
Similar conceptually to 978b2f65aa and building upon
the infrastructure created by that commit.

This reverts commit ccce90b398.  This
optimization is unsafe, at least, of rollbacks and rollbacks to
savepoints, but I'm concerned there may be other problematic cases as
well.  Therefore, I've decided to revert this pending further
investigation.

Robert Haas ccce90b398 Use group updates when setting transaction status in clog.
Commit 0e141c0fbb introduced a mechanism
to reduce contention on ProcArrayLock by having a single process clear
XIDs in the procArray on behalf of multiple processes, reducing the
need to hand the lock around.  Use a similar mechanism to reduce
contention on CLogControlLock.  Testing shows that this very
significantly reduces the amount of time waiting for CLogControlLock
on high-concurrency pgbench tests run on a large multi-socket
machines; whether that translates into a TPS improvement depends on
how much of that contention is simply shifted to some other lock,
particularly WALWriteLock.

tested, and benchmarked over a period of about 15 months by Simon
Riggs, Robert Haas, Andres Freund, Jesper Pedersen, and especially by
Tomas Vondra and Dilip Kumar.

Heikki Linnakangas bb38fb0d43 Fix race condition in preparing a transaction for two-phase commit.
To lock a prepared transaction's shared memory entry, we used to mark it
with the XID of the backend. When the XID was no longer active according
to the proc array, the entry was implicitly considered as not locked
anymore. However, when preparing a transaction, the backend's proc array
entry was cleared before transfering the locks (and some other state) to
the prepared transaction's dummy PGPROC entry, so there was a window where
another backend could finish the transaction before it was in fact fully
prepared.

To fix, rewrite the locking mechanism of global transaction entries. Instead
of an XID, just have simple locked-or-not flag in each entry (we store the
locking backend's backend id rather than a simple boolean, but that's just
for debugging purposes). The backend is responsible for explicitly unlocking
the entry, and to make sure that that happens, install a callback to unlock
it on abort or process exit.

Backpatch to all supported versions.

Simon Riggs efc16ea520 Allow read only connections during recovery, known as Hot Standby.
Enabled by recovery_connections = on (default) and forcing archive recovery using a recovery.conf. Recovery processing now emulates the original transactions as they are replayed, providing full locking and MVCC behaviour for read only queries. Recovery must enter consistent state before connections are allowed, so there is a delay, typically short, before connections succeed. Replay of recovering transactions can conflict and in some cases deadlock with queries during recovery; these result in query cancellation after max_standby_delay seconds have expired. Infrastructure changes have minor effects on normal running, though introduce four new types of WAL record.

New test mode "make standbycheck" allows regression tests of static command behaviour on a standby server while in recovery. Typical and extreme dynamic behaviours have been checked via code inspection and manual testing. Few port specific behaviours have been utilised, though primary testing has been on Linux only so far.

This commit is the basic patch. Additional changes will follow in this release to enhance some aspects of behaviour, notably improved handling of conflicts, deadlock detection and query cancellation. Changes to VACUUM FULL are also required.

Heikki Linnakangas cd87b6f8a5 Fix an old bug in multixact and two-phase commit. Prepared transactions can
be part of multixacts, so allocate a slot for each prepared transaction in
the "oldest member" array in multixact.c. On PREPARE TRANSACTION, transfer
the oldest member value from the current backends slot to the prepared xact
slot. Also save and recover the value from the 2pc state file.

The symptom of the bug was that after a transaction prepared, a shared lock
still held by the prepared transaction was sometimes ignored by other
transactions.

Fix back to 8.1, where both 2PC and multixact were introduced.

Tom Lane 0007490e09 Convert the arithmetic for shared memory size calculation from 'int'
to 'Size' (that is, size_t), and install overflow detection checks in it.
This allows us to remove the former arbitrary restrictions on NBuffers
etc.  It won't make any difference in a 32-bit machine, but in a 64-bit
machine you could theoretically have terabytes of shared buffers.
(How efficiently we could manage 'em remains to be seen.)  Similarly,
num_temp_buffers, work_mem, and maintenance_work_mem can be set above
2Gb on a 64-bit machine.  Original patch from Koichi Suzuki, additional
work by moi.

Tom Lane 7762619e95 Replace pg_shadow and pg_group by new role-capable catalogs pg_authid
and pg_auth_members.  There are still many loose ends to finish in this
patch (no documentation, no regression tests, no pg_dump support for
instance).  But I'm going to commit it now anyway so that Alvaro can
make some progress on shared dependencies.  The catalog changes should
be pretty much done.

Tom Lane e26b0abda3 Arrange to fsync two-phase-commit state files only during checkpoints;
given reasonably short lifespans for prepared transactions, this should
mean that only a small minority of state files ever need to be fsynced
at all.  Per discussion with Heikki Linnakangas.

Tom Lane a8d1075f27 Add a time-of-preparation column to the pg_prepared_xacts view, per an
old suggestion by Oliver Jowett.  Also, add a transaction column to the
pg_locks view to show the xid of each transaction holding or awaiting
locks; this allows prepared transactions to be properly associated with
the locks they own.  There was already a column named 'transaction',
and I chose to rename it to 'transactionid' --- since this column is
new in the current devel cycle there should be no backwards compatibility
issue to worry about.
