Add $PostgreSQL$ markers to a lot of files that were missing them.
This particular batch was just for *.c and *.h file.
The changes were made with the following 2 commands:

Fix some makefiles that fail to yield good results from 'make -qp'.
This doesn't really matter for ordinary building of Postgres, but it's
useful for automated checks, such as my just-committed pgcheckdefines.

Be careful to include postgres.h *before* any system headers, to ensure
that the right flavors of largefile-related definitions are seen.
Most of these changes are probably unnecessary, but better safe than
sorry.

Fix relative path references so that make knowns which dependencies refer
to one another. Sort out builddir vs srcdir variable namings. Remove some
now obsoleted make variables.

Generated header files parse.h and fmgroids.h are now copied into
the src/include tree, so that -I backend is no longer necessary anywhere.
Also, clean up some bit rot in contrib tree.
