Generally if a role is granted membership to another role with NOINHERIT
they must use SET ROLE to access the privileges of that role, however
with predefined roles the membership and privilege is conflated. Fix that
by replacing is_member_of_role with has_privs_for_role for predefined
roles. Patch does not remove is_member_of_role from acl.h, but it does
add a warning not to use that function for privilege checking. Not
backpatched based on hackers list discussion.

This moves the functions related to performing WAL recovery into the new
xlogrecovery.c source file, leaving xlog.c responsible for maintaining
the WAL buffers, coordinating the startup and switch from recovery to
normal operations, and other miscellaneous stuff that have always been in
xlog.c.

All such code deals with this global variable in one of three ways.
Sometimes the same functions use it in more than one of these ways
at the same time.
First, sometimes it's an implicit argument to one or more functions
being called in xlog.c or elsewhere, and must be set to the
appropriate value before calling those functions lest they
misbehave. In those cases, it is now passed as an explicit argument
instead.
Second, sometimes it's used to obtain the current timeline after
the end of recovery, i.e. the timeline to which WAL is being
written and flushed. Such code now calls GetWALInsertionTimeLine()
or relies on the new out parameter added to GetFlushRecPtr().
Third, sometimes it's used during recovery to store the current
replay timeline. That can change, so such code must generally
update the value before each use. It can still do that, but must
now use a local variable instead.
The net effect of these changes is to reduce by a fair amount the
amount of code that is directly accessing this global variable.
That's good, because history has shown that we don't always think
clearly about which timeline ID it's supposed to contain at any
given point in time, or indeed, whether it has been or needs to
be initialized at any given point in the code.

Previously, walreceiver always closed the currently-opened WAL segment
and created its archive notification file, after it finished writing
the current segment up and received any WAL data that should be
written into the next segment. If walreceiver exited just before
any WAL data in the next segment arrived at standby, it did not
create the archive notification file of the current segment
even though that's known completed. This behavior could cause
WAL archiving of the segment to be delayed until subsequent
restartpoints or checkpoints created its notification file.
To fix the issue, this commit changes walreceiver so that it creates
an archive notification file of a current WAL segment immediately
if that's known completed before receiving next WAL data.
Back-patch to all supported branches.

WAL records may span multiple segments, but XLogWrite() does not
wait for the entire record to be written out to disk before
creating archive status files. Instead, as soon as the last WAL page of
the segment is written, the archive status file is created, and the
archiver may process it. If PostgreSQL crashes before it is able to
write and flush the rest of the record (in the next WAL segment), the
wrong version of the first segment file lingers in the archive, which
causes operations such as point-in-time restores to fail.
To fix this, keep track of records that span across segments and ensure
that segments are only marked ready-for-archival once such records have
been completely written to disk.
This has always been wrong, so backpatch all the way back.

To add support for streaming transactions at prepare time into the
built-in logical replication, we need to do the following things:
* Modify the output plugin (pgoutput) to implement the new two-phase API
callbacks, by leveraging the extended replication protocol.
* Modify the replication apply worker, to properly handle two-phase
transactions by replaying them on prepare.
* Add a new SUBSCRIPTION option "two_phase" to allow users to enable
two-phase transactions. We enable the two_phase once the initial data sync
is over.
We however must explicitly disable replication of two-phase transactions
during replication slot creation, even if the plugin supports it. We
don't need to replicate the changes accumulated during this phase,
and moreover, we don't have a replication connection open so we don't know
where to send the data anyway.
The streaming option is not allowed with this new two_phase option. This
can be done as a separate patch.
We don't allow to toggle two_phase option of a subscription because it can
lead to an inconsistent replica. For the same reason, we don't allow to
refresh the publication once the two_phase is enabled for a subscription
unless copy_data option is false.

Before a restartpoint finishes PreallocXlogFiles(), a startup process
KeepFileRestoredFromArchive() call can unlink the preallocated segment.
If a CHECKPOINT sql command had elicited the restartpoint experiencing
the race condition, that sql command failed. Moreover, the restartpoint
omitted its log_checkpoints message and some inessential resource
reclamation. Prevent the ERROR by skipping open() of the segment.
Since these consequences are so minor, no back-patch.

I started out with the goal of reporting ERRCODE_CONNECTION_FAILURE
when walrcv_connect() fails, but as I looked around I realized that
whoever wrote this code was of the opinion that errcodes are purely
optional. That's not my understanding of our project policy. Hence,
make sure that an errcode is provided in each ereport that (a) is
ERROR or higher level and (b) isn't arguably an internal logic error.
Also fix some very dubious existing errcode assignments.
While this is not per policy, it's also largely cosmetic, since few
of these cases could get reported to applications. So I don't
feel a need to back-patch.

The only change worthy of note is that pgindent messed up the formatting
of launcher.c's struct LogicalRepWorkerId, which led me to notice that
that struct wasn't used at all anymore, so I just took it out.
Commit 2c8dd05d6c added the atomic variable writtenUpto into
walreceiver's shared memory information. It's initialized only
when walreceiver started up but could be read via pg_stat_wal_receiver
view anytime, i.e., even before it's initialized. In the server built
with --disable-atomics and --disable-spinlocks, this uninitialized
atomic variable read could cause "invalid spinlock number: 0" error.
This commit changed writtenUpto so that it's initialized at
the postmaster startup, to avoid the uninitialized variable read
via pg_stat_wal_receiver and fix the error.
Also this commit moved the read of writtenUpto after the release
of spinlock protecting walreceiver's shared variables. This is
necessary to prevent new spinlock from being taken by atomic
variable read while holding another spinlock, and to shorten
the spinlock duration. This change leads writtenUpto not to be
consistent with the other walreceiver's shared variables protected
by a spinlock. But this is OK because writtenUpto should not be
used for data integrity checks.
Back-patch to v13 where commit 2c8dd05d6c introduced the bug.

While ereport() and elog() themselves are quite cheap when the
error message level is too low to be printed, some places need to do
substantial work before they can call those macros at all. To allow
optimizing away such setup work when nothing is to be printed, make
elog.c export a new function message_level_is_interesting(elevel)
that reports whether ereport/elog will do anything. Make use of that
in various places that had ad-hoc direct tests of log_min_messages etc.
Also teach ProcSleep to use it to avoid some work. (There may well
be other places that could usefully use this; I didn't search hard.)
Within elog.c, refactor a little bit to avoid having duplicate copies
of the policy-setting logic. When that code was written, we weren't
relying on the availability of inline functions; so it had some
duplications in the name of efficiency, which I got rid of.

Commit 1e53fe0e70 changed background processes so that they use
standard SIGHUP handler. Like that, this commit makes walreceiver
use standard SIGHUP and SIGTERM handlers, to simplify the code.
As the side effect of this commit, walreceiver can wake up and process
the configuration files promptly when receiving SIGHUP. Because the
standard SIGHUP handler sets the latch. On the other hand, previously
there could be a time lag between the receipt of SIGHUP and
the process of configuration files since the dedicated handler didn't
set the latch.

Previously the standby server didn't archive timeline history files
streamed from the primary even when archive_mode is set to "always",
while it archives the streamed WAL files. This could cause the PITR to
fail because there was no required timeline history file in the archive.
The cause of this issue was that walreceiver didn't mark those files as
ready for archiving.
This commit makes walreceiver mark those streamed timeline history
files as ready for archiving if archive_mode=always. Then the archiver
process archives the marked timeline history files.
Back-patch to all supported versions.

We decided that the policy established in commit 7634bd4f6 for
the bgwriter, checkpointer, walwriter, and walreceiver processes,
namely that they should accept SIGQUIT at all times, really ought
to apply uniformly to all postmaster children. Therefore, get
rid of the duplicative and inconsistent per-process code for
establishing that signal handler and removing SIGQUIT from BlockSig.
Instead, make InitPostmasterChild do it.
The handler set up by InitPostmasterChild is SignalHandlerForCrashExit,
which just summarily does _exit(2). In interactive backends, we
almost immediately replace that with quickdie, since we would prefer
to try to tell the client that we're dying. However, this patch is
changing the behavior of autovacuum (both launcher and workers), as
well as walsenders. Those processes formerly also used quickdie,
but AFAICS that was just mindless copy-and-paste: they don't have
any interactive client that's likely to benefit from being told this.
The stats collector continues to be an outlier, in that it thinks
SIGQUIT means normal exit. That should probably be changed for
consistency, but there's another patch set where that's being
dealt with, so I didn't do so here.

The bgwriter, checkpointer, walwriter, and walreceiver processes
claimed to allow SIGQUIT "at all times". In reality SIGQUIT
would get re-blocked during error recovery, because we didn't
update the actual signal mask immediately, so sigsetjmp() would
save and reinstate a mask that includes SIGQUIT.
This appears to be simply a coding oversight. There's never a
good reason to hold off SIGQUIT in these processes, because it's
going to just call _exit(2) which should be safe enough, especially
since the postmaster is going to tear down shared memory afterwards.
Hence, stick in PG_SETMASK() calls to install the modified BlockSig
mask immediately.
Also try to improve the comments around sigsetjmp blocks. Most of
them were just referencing postgres.c, which is misleading because
actually postgres.c manages the signals differently.
No back-patch, since there's no evidence that this is causing any
problems in the field.

To make GetSnapshotData() more scalable, it cannot not look at at each proc's
xmin: While snapshot contents do not need to change whenever a read-only
transaction commits or a snapshot is released, a proc's xmin is modified in
those cases. The frequency of xmin modifications leads to, particularly on
higher core count systems, many cache misses inside GetSnapshotData(), despite
the data underlying a snapshot not changing. That is the most
significant source of GetSnapshotData() scaling poorly on larger systems.
Without accessing xmins, GetSnapshotData() cannot calculate accurate horizons /
thresholds as it has so far. But we don't really have to: The horizons don't
actually change that much between GetSnapshotData() calls. Nor are the horizons
actually used every time a snapshot is built.
The trick this commit introduces is to delay computation of accurate horizons
until there use and using horizon boundaries to determine whether accurate
horizons need to be computed.
The use of RecentGlobal[Data]Xmin to decide whether a row version could be
removed has been replaces with new GlobalVisTest* functions. These use two
thresholds to determine whether a row can be pruned:
1) definitely_needed, indicating that rows deleted by XIDs >= definitely_needed
are definitely still visible.
2) maybe_needed, indicating that rows deleted by XIDs < maybe_needed can
definitely be removed
GetSnapshotData() updates definitely_needed to be the xmin of the computed
snapshot.
When testing whether a row can be removed (with GlobalVisTestIsRemovableXid())
and the tested XID falls in between the two (i.e. XID >= maybe_needed && XID <
definitely_needed) the boundaries can be recomputed to be more accurate. As it
is not cheap to compute accurate boundaries, we limit the number of times that
happens in short succession. As the boundaries used by
GlobalVisTestIsRemovableXid() are never reset (with maybe_needed updated by
GetSnapshotData()), it is likely that further test can benefit from an earlier
computation of accurate horizons.
To avoid regressing performance when old_snapshot_threshold is set (as that
requires an accurate horizon to be computed), heap_page_prune_opt() doesn't
unconditionally call TransactionIdLimitedForOldSnapshots() anymore. Both the
computation of the limited horizon, and the triggering of errors (with
SetOldSnapshotThresholdTimestamp()) is now only done when necessary to remove
tuples.
This commit just removes the accesses to PGXACT->xmin from
GetSnapshotData(), but other members of PGXACT residing in the same
cache line are accessed. Therefore this in itself does not result in a
significant improvement. Subsequent commits will take advantage of the
fact that GetSnapshotData() now does not need to access xmins anymore.
Note: This contains a workaround in heap_page_prune_opt() to keep the
snapshot_too_old tests working. While that workaround is ugly, the tests
currently are not meaningful, and it seems best to address them separately.

d140f2f3 has renamed receivedUpto to flushedUpto, and has added
writtenUpto to the WAL receiver's shared memory information, but
pg_stat_wal_receiver was not consistent with that. This commit renames
received_lsn to flushed_lsn, and adds a new column called written_lsn.

GetWalRcvWriteRecPtr() previously reported the latest *flushed*
location. Adopt the conventional terminology used elsewhere in the tree
by renaming it to GetWalRcvFlushRecPtr(), and likewise for some related
variables that used the term "received".
Add a new definition of GetWalRcvWriteRecPtr(), which returns the latest
*written* value. This will allow later patches to use the value for
non-data-integrity purposes, without having to wait for the flush
pointer to advance.

The definitions of the routines defined in xlogarchive.c have been part
of xlog_internal.h which is included by several frontend tools, but all
those routines are only called by the backend. More cleanup could be
done within xlog_internal.h, but that's already a nice cut.
This will help a follow-up patch for pg_rewind where handling of
restore_command is added for frontends.

The parameters primary_conninfo, primary_slot_name and
wal_receiver_create_temp_slot can now be changed with a simple "reload"
signal, no longer requiring a server restart. This is achieved by
signalling the walreceiver process to terminate and having it start
again with the new values.

Commit 3297308278 gave walreceiver the ability to create and use a
temporary replication slot, and made it controllable by a GUC (enabled
by default) that can be changed with SIGHUP. That's useful but has two
problems: one, it's possible to cause the origin server to fill its disk
if the slot doesn't advance in time; and also there's a disconnect
between state passed down via the startup process and GUCs that
walreceiver reads directly.
We handle the first problem by setting the option to disabled by
default. If the user enables it, its on their head to make sure that
disk doesn't fill up.
We handle the second problem by passing the flag via startup rather than
having walreceiver acquire it directly, and making it PGC_POSTMASTER
(which ensures a walreceiver always has the fresh value). A future
commit can relax this (to PGC_SIGHUP again) by having the startup
process signal walreceiver to shutdown whenever the value changes.

The init_ps_display() arguments were mostly lies by now, so to match
typical usage, just use one argument and let the caller assemble it
from multiple sources if necessary. The only user of the additional
arguments is BackendInitialize(), which was already doing string
assembly on the caller side anyway.
Remove the second argument of set_ps_display() ("force") and just
handle that in init_ps_display() internally.
BackendInitialize() also used to set the initial status as
"authentication", but that was very far from where authentication
actually happened. So now it's set to "initializing" and then
"authentication" just before the actual call to
ClientAuthentication().

parser/parse_node.h explicitly where they previously got it indirectly
somehow.

nodes/parsenodes.h.

If no permanent replication slot is configured using
primary_slot_name, the walreceiver now creates and uses a temporary
replication slot. A new setting wal_receiver_create_temp_slot can be
used to disable this behavior, for example, if the remote instance is
out of replication slots.

A new function EmitProcSignalBarrier() can be used to emit a global
barrier which all backends that participate in the ProcSignal
mechanism must absorb, and a new function WaitForProcSignalBarrier()
can be used to wait until all relevant backends have in fact
absorbed the barrier.
This can be used to coordinate global state changes, such as turning
checksums on while the system is running.
There's no real client of this mechanism yet, although two are
proposed, but an enum has to have at least one element, so this
includes a placeholder type (PROCSIGNAL_BARRIER_PLACEHOLDER) which
should be replaced by the first real client of this mechanism to
get committed.

Where possible, share signal handler code and main loop interrupt
checking. This saves quite a bit of code and should simplify
maintenance, too.
This commit intends not to change the way anything works, even
though that might allow more code to be unified. It does unify
a bunch of individual variables into a ShutdownRequestPending
flag that has is now used by a bunch of different process types,
though.

XLogFileNameP() is a wrapper routine able to build a palloc'd string for
a WAL segment name, which is used for error string generation. There
were several code paths where it gets called in a critical section,
where memory allocation is not allowed. This results in triggering
an assertion failure instead of generating the wanted error message.
Another, more annoying, problem is that if the allocation to generate
the WAL segment name fails on OOM, then the failure would be escalated
to a PANIC.
This removes the routine and all its callers are replaced with a logic
using a fixed-size buffer. This way, all the existing mistakes are
fixed and future ones are prevented.

AuxiliaryProcessMain does ProcSignalInit, so one might expect that
auxiliary processes would need to respond to SendProcSignal, but none
of the auxiliary processes do that. Change them to use
procsignal_sigusr1_handler instead of their own private handlers so
that they do. Besides seeming more correct, this is also less code. It
shouldn't make any functional difference right now because, as far as
we know, there are no current cases where SendProcSignal targets an
auxiliary process, but there are plans to change that in the future.

This is quite unsafe, even for the case of ereport(FATAL) where we won't
return control to the interrupted code, and despite this code's use of
a flag to restrict the areas where we'd try to do it. It's possible
for example that we interrupt malloc or free while that's holding a lock
that's meant to protect against cross-thread interference. Then, any
attempt to do malloc or free within ereport() will result in a deadlock,
preventing the walreceiver process from exiting in response to SIGTERM.
We hypothesize that this explains some hard-to-reproduce failures seen
in the buildfarm.
Hence, get rid of the immediate-exit code in WalRcvShutdownHandler,
as well as the logic associated with WalRcvImmediateInterruptOK.
Instead, we need to take care that potentially-blocking operations
in the walreceiver's data transmission logic (libpqwalreceiver.c)
will respond reasonably promptly to the process's latch becoming
set and then call ProcessWalRcvInterrupts. Much of the needed code
for that was already present in libpqwalreceiver.c. I refactored
things a bit so that all the uses of PQgetResult use latch-aware
waiting, but didn't need to do much more.
These changes should be enough to ensure that libpqwalreceiver.c
will respond promptly to SIGTERM whenever it's waiting to receive
data. In principle, it could block for a long time while waiting
to send data too, and this patch does nothing to guard against that.
I think that that hazard is mostly theoretical though: such blocking
should occur only if we fill the kernel's data transmission buffers,
and we don't generally send enough data to make that happen without
waiting for input. If we find out that the hazard isn't just
theoretical, we could fix it by using PQsetnonblocking, but that
would require more ticklish changes than I care to make now.
This is a bug fix, but it seems like too big a change to push into
the back branches without much more testing than there's time for
right now. Perhaps we'll back-patch once we have more confidence
in the change.

Instead of inferring epoch progress from xids and checkpoints,
introduce a 64 bit FullTransactionId type and use it to track xid
generation. This fixes an unlikely bug where the epoch is reported
incorrectly if the range of active xids wraps around more than once
between checkpoints.
The only user-visible effect of this commit is to correct the epoch
used by txid_current() and txid_status(), also visible with
pg_controldata, in those rare circumstances. It also creates some
basic infrastructure so that later patches can use 64 bit
transaction IDs in more places.
The new type is a struct that we pass by value, as a form of strong
typedef. This prevents the sort of accidental confusion between
TransactionId and FullTransactionId that would be possible if we
were to use a plain old uint64.
