fa42c2ecb0 docs: Remove notes about incompatibilies with very old versions.
These are old enough that they'll cause more confusion and distraction
to new readers, than they could help anyone upgrade from very old
servers.

7db0cde6b5 Revert "Avoid the creation of the free space map for small heap relations".
This feature was using a process local map to track the first few blocks
in the relation.  The map was reset each time we get the block with enough
freespace.  It was discussed that it would be better to track this map on
a per-relation basis in relcache and then invalidate the same whenever
vacuum frees up some space in the page or when FSM is created.  The new
design would be better both in terms of API design and performance.

29d108cdec Doc: Update the documentation for FSM behavior for small tables.
In commit b0eaa4c51b, we have avoided the creation of FSM for small tables.
So the functions that use FSM to compute the free space can return zero for
such tables.  This was previously also possible for the cases where the
vacuum has not been triggered to update FSM.

This commit updates the comments in code and documentation to reflect this
behavior.

c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

25fff40798 Default monitoring roles
Three nologin roles with non-overlapping privs are created by default
* pg_read_all_settings - read all GUCs.
* pg_read_all_stats - pg_stat_*, pg_database_size(), pg_tablespace_size()
* pg_stat_scan_tables - may lock/scan tables

Top level role - pg_monitor includes all of the above by default, plus others

3a9d430af5 doc: Fix DocBook XML validity
The main problem is that DocBook SGML allows indexterm elements just
about everywhere, but DocBook XML is stricter.  For example, this common
pattern

    <varlistentry>
     <indexterm>...</indexterm>
     <term>...</term>
     ...
    </varlistentry>

needs to be changed to something like

    <varlistentry>
     <term>...<indexterm>...</indexterm></term>
     ...
    </varlistentry>

There is currently nothing in the build system that enforces that things
stay valid, because that requires additional tools and will receive
separate consideration.

66424a2848 Fix indentation of verbatim block elements
Block elements with verbatim formatting (literallayout, programlisting,
screen, synopsis) should be aligned at column 0 independent of the surrounding
SGML, because whitespace is significant, and indenting them creates erratic
whitespace in the output.  The CSS stylesheets already take care of indenting
the output.

Assorted markup improvements to go along with it.

6736da5484 Make the blkno arguments bigints instead of int4s. A signed int4 is not
large enough for block numbers higher than 2^31. The old pre-FSM-rewrite
pg_freespacemap implementation got this right. While we're at it, remove
some unnecessary #includes.

15c121b3ed Rewrite the FSM. Instead of relying on a fixed-size shared memory segment, the
free space information is stored in a dedicated FSM relation fork, with each
relation (except for hash indexes; they don't use FSM).

This eliminates the max_fsm_relations and max_fsm_pages GUC options; remove any
trace of them from the backend, initdb, and documentation.

Rewrite contrib/pg_freespacemap to match the new FSM implementation. Also
introduce a new variant of the get_raw_page(regclass, int4, int4) function in
contrib/pageinspect that let's you to return pages from any relation fork, and
a new fsm_page_contents() function to inspect the new FSM pages.

3b3251cb95 Ensure that all <sect1> and <refentry> tags have IDs. This is needed
to ensure that the resulting webpages have predictable URLs, instead of
ever-changing numeric IDs.  The new contrib docs were the biggest
offender, but some old stuff had the problem too.  Also, rename a couple
of new contrib sgml files for consistency's sake.
