The original titles only had the module name, which is not very useful
when scanning the list. By adding a very brief description to each
title, the table of contents becomes friendlier.
Also amend the introduction in the "additional modules" appendix, using
the word "Extension" more extensively. Nowadays, almost all contrib
modules are extensions, so this is also helpful.

Historically we've been lax about this, but seeing that we're not
lax in C files, there doesn't seem to be a good reason to be so
in the documentation. Remove the existing occurrences (mostly
though not entirely in copied-n-pasted psql output), and modify
.gitattributes so that "git diff --check" will warn about future
cases.
While at it, add *.pm to the set of extensions .gitattributes
knows about, and remove some obsolete entries for files that
we don't have in the tree anymore.

In one or two places it seemed reasonable to modify the example so as
to shorten its output slightly; but for the most part I just added a
&zwsp; after 67 characters, which is the most we can fit on a line
of monospace text in A4 format.

Update links that resulted in redirects. Most are changes from http to
https, but there are also some other minor edits. (There are still some
redirects where the target URL looks less elegant than the one we
currently have. I have left those as is.)
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name. Add a warning option to catch future
occurrences.

Per discussion, this is a more understandable and future-proof way of
exposing the setting to users. On-disk, we can still store it in words,
so as to not break on-disk compatibility with beta1.
Along the way, clean up the code associated with Bloom reloptions.
Provide explicit macros for default and maximum lengths rather than
having magic numbers buried in multiple places in the code. Drop
the adjustBloomOptions() code altogether: it was useless in view of
the fact that reloptions.c already performed default-substitution and
range checking for the options. Rename a couple of macros and types
for more clarity.

Module provides new access method. It is actually a simple Bloom filter
implemented as pgsql's index. It could give some benefits on search
with large number of columns.
Module is a single way to test generic WAL interface committed earlier.
