If an operation being waited for does not complete within the timeout,
then exit with a nonzero exit status. This was previously handled
inconsistently.
Commit 05cd12ed5 ("pg_ctl: Change default to wait for all actions")
was a tad sloppy about updating the documentation to match. The
documentation was also sorely in need of a copy-editing pass, having
been adjusted at different times by different people who took little
care to maintain consistency of style.
The different actions in pg_ctl had different defaults for -w and -W,
mostly for historical reasons. Most users will want the -w behavior, so
make that the default.
Remove the -w option in most example and test code, so avoid confusion
and reduce verbosity. pg_upgrade is not touched, so it can continue to
work with older installations.

Various example and test code used -m fast explicitly, but since it's
the default, this can be omitted now or should be replaced by a better
example.
pg_upgrade is not touched, so it can continue to operate with older
installations.
When waiting is selected for the promote action, look into pg_control
until the state changes, then use the PQping-based waiting until the
server is reachable.

Many automated test suites call pg_ctl. Buildfarm members axolotl,
hornet, mandrill, shearwater, sungazer and tern have failed when server
shutdown took longer than the pg_ctl default 60s timeout. This addition
permits slow hosts to easily raise the timeout without us editing a
--timeout argument into every test suite pg_ctl call. Back-patch to 9.1
(all supported versions) for the sake of automated testing.

pg_ctl will log to the Windows event log when it is running as a service,
which is the primary way of running PostgreSQL on Windows. This option
makes it possible to specify which event source to use for this, in order
to separate different instances. The server logging itself is still controlled
by the regular logging parameters, including a separate setting for the event
source. The parameter to pg_ctl only controlls the logging from pg_ctl itself.

Return '4' and report a meaningful error message when a non-existent or
invalid data directory is passed. Previously, pg_ctl would just report
the server was not running.

DocBook XML is superficially compatible with DocBook SGML but has a
slightly stricter DTD that we have been violating in a few cases.
Although XSLT doesn't care whether the document is valid, the style
sheets don't necessarily process invalid documents correctly, so we need
to work toward fixing this.
This first commit moves the indexterms in refentry elements to an
allowed position. It has no impact on the output.
This changes the behavior of the start and stop actions to exit
successfully if the server was already started or stopped.
This changes the default behavior of the start action: Before, if the
server was already running, it would print a message and succeed. Now,
that situation will result in an error. When running in idempotent
mode, no message is printed and pg_ctl exits successfully.
It was considered to just make the idempotent behavior the default and
only option, but pg_upgrade needs the old behavior.
Before, some places didn't document the short options (-? and -V),
some documented both, some documented nothing, and they were listed in
various orders. Now this is hopefully more consistent and complete.
The default for the choice attribute of the <arg> element is "opt",
which would normally put the argument inside brackets. But the DSSSL
stylesheets contain a hack that treats <arg> directly inside <group>
specially, so that <group><arg>-x</arg><arg>-y</arg></group> comes out
as [ -x | -y ] rather than [ [-x] | [-y] ], which it would technically
be. But when building man pages, this doesn't work, and so the
command synopses on the man pages contain lots of extra brackets.
By putting choice="opt" or choice="plain" explicitly on every <arg>
and <group> element, we avoid any toolchain dependencies like that,
and it also makes it clearer in the source code what is meant.
In passing, make some small corrections in the documentation about
which arguments are really optional or not.
Add more markup in particular so that the command options appear
consistently in monospace in the HTML output.
On the vacuumdb reference page, remove listing all the possible
options in the synopsis. They have become too many now; we have the
detailed options list for that.
There is what may actually be a mistake in our markup. The problem is
in a situation like
<para>
<command>FOO</command> is ...
there is strictly speaking a line break before "FOO". In the HTML
output, this does not appear to be a problem, but in the man page
output, this shows up, so you get double blank lines at odd places.
So far, we have attempted to work around this with an XSL hack, but
that causes other problems, such as creating run-ins in places like
<acronym>SQL</acronym> <command>COPY</command>
So fix the problem properly by removing the extra whitespace. I only
fixed the problems that affect the man page output, not all the
places.
Since start/stop/restart/reload/status is a kind of standard command
set, it seems odd to insert the special-purpose "promote" in between
the closely related "restart" and "reload". So put it after "status"
in code and documentation.
Put the documentation of the -U option in some sensible place.
Rewrite the synopsis sentence in help and documentation to make it
less of a growing mouthful.
Explicitly document that the -o options of pg_ctl init mode are meant
for initdb, not postgres (Euler Taveira de Oliveira). Assorted other
copy-editing (Tom).
Command synopses using <cmdsynopsis> with multiple variants previously used
<sbr> to break lines between variants. The new man page toolchain introduced
in 9.0 makes a mess out of that, and that markup was probably wrong all along,
because <sbr> is supposed to break lines within a synopsis, not between them.
So fix that by using multiple <cmdsynopsis> elements inside <refsynopsisdiv>.

At present, killing the startup process does not release any locks it holds,
so we must wait to stop the startup and walreceiver processes until all
read-only backends have exited. Without this patch, the startup and
walreceiver processes never exit, so the server gets permanently stuck in
a half-shutdown state.

The endterm attribute is mainly useful when the toolchain does not support
automatic link target text generation for a particular situation. In the
past, this was required by the man page tools for all reference page links,
but that is no longer the case, and it now actually gets in the way of
proper automatic link text generation. The only remaining use cases are
currently xrefs to refsects.
pg_ctl gets a new mode that runs initdb. Adjust the documentation a bit to
not assume that initdb is the only way to run database cluster initialization.
But don't replace initdb as the canonical way.
