2dedf4d9a8 Integrate recovery.conf into postgresql.conf
recovery.conf settings are now set in postgresql.conf (or other GUC
sources).  Currently, all the affected settings are PGC_POSTMASTER;
this could be refined in the future case by case.

Recovery is now initiated by a file recovery.signal.  Standby mode is
initiated by a file standby.signal.  The standby_mode setting is
gone.  If a recovery.conf file is found, an error is issued.

The trigger_file setting has been renamed to promote_trigger_file as
part of the move.

The documentation chapter "Recovery Configuration" has been integrated
into "Server Configuration".

pg_basebackup -R now appends settings to postgresql.auto.conf and
creates a standby.signal file.

3c49c6facb Convert documentation to DocBook XML
Since some preparation work had already been done, the only source
changes left were changing empty-element tags like <xref linkend="foo">
to <xref linkend="foo"/>, and changing the DOCTYPE.

The source files are still named *.sgml, but they are actually XML files
now.  Renaming could be considered later.

In the build system, the intermediate step to convert from SGML to XML
is removed.  Everything is build straight from the source files again.
The OpenSP (or the old SP) package is no longer needed.

The documentation toolchain instructions are updated and are much
simpler now.

c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

f10637ebe0 Replace references to "xlog" with "wal" in docs.
Commit f82ec32ac3 renamed the pg_xlog
directory to pg_wal. To make things consistent, we decided to eliminate
"xlog" from user-visible docs.

bb4eefe7bf doc: Improve DocBook XML validity
DocBook XML is superficially compatible with DocBook SGML but has a
slightly stricter DTD that we have been violating in a few cases.
Although XSLT doesn't care whether the document is valid, the style
sheets don't necessarily process invalid documents correctly, so we need
to work toward fixing this.

This first commit moves the indexterms in refentry elements to an
allowed position.  It has no impact on the output.

bb7520cc26 Make documentation of --help and --version options more consistent
Before, some places didn't document the short options (-? and -V),
some documented both, some documented nothing, and they were listed in
various orders.  Now this is hopefully more consistent and complete.

1c882e07da Split contrib documentation into extensions and programs
Create separate appendixes for contrib extensions and other server
plugins on the one hand, and utility programs on the other.  Recast
the documentation of the latter as refentries, so that man pages are
generated.

66424a2848 Fix indentation of verbatim block elements
Block elements with verbatim formatting (literallayout, programlisting,
screen, synopsis) should be aligned at column 0 independent of the surrounding
SGML, because whitespace is significant, and indenting them creates erratic
whitespace in the output.  The CSS stylesheets already take care of indenting
the output.

Assorted markup improvements to go along with it.

8fd733bd19 Disable pg_standby -l option because the backend doesn't expect the recovered
file to be a symlink. We tried to fix this issue with an earlier server-side
patch, but it didn't fix the whole issue.

The same bug is present in older releases as well, but the 8.4 train is
about to leave the station, and I'm not sure if have consensus on whether
we can remove the -l option in back-branches or do we need to attempt a
server-side fix to make symlinking safe.

9e403c2587 Add recovery_end_command option to recovery.conf. recovery_end_command
is run at the end of archive recovery, providing a chance to do external
cleanup. Modify pg_standby so that it no longer removes the trigger file,
that is to be done using the recovery_end_command now.

Provide a "smart" failover mode in pg_standby, where we don't fail over
immediately, but only after recovering all unapplied WAL from the archive.
That gives you zero data loss assuming all WAL was archived before
failover, which is what most users of pg_standby actually want.

3b3251cb95 Ensure that all <sect1> and <refentry> tags have IDs. This is needed
to ensure that the resulting webpages have predictable URLs, instead of
ever-changing numeric IDs.  The new contrib docs were the biggest
offender, but some old stuff had the problem too.  Also, rename a couple
of new contrib sgml files for consistency's sake.
