Add a column collprovider to pg_collation that determines which library
provides the collation data. The existing choices are default and libc,
and this adds an icu choice, which uses the ICU4C library.
The pg_locale_t type is changed to a union that contains the
provider-specific locale handles. Users of locale information are
changed to look into that struct for the appropriate handle to use.
Also add a collversion column that records the version of the collation
when it is created, and check at run time whether it is still the same.
This detects potentially incompatible library upgrades that can corrupt
indexes and other structures. This is currently only supported by
ICU-provided collations.
initdb initializes the default collation set as before from the `locale
-a` output but also adds all available ICU locales with a "-x-icu"
appended.
Currently, ICU-provided collations can only be explicitly named
collations. The global database locales are still always libc-provided.
ICU support is enabled by configure --with-icu.

The latter was already the dominant use, and it's preferable because
in C the convention is that intXX means XX bits. Therefore, allowing
mixed use of int2, int4, int8, int16, int32 is obviously confusing.
Remove the typedefs for int2 and int4 for now. They don't seem to be
widely used outside of the PostgreSQL source tree, and the few uses
can probably be cleaned up by the time this ships.
Install just one instance of the "C" and "POSIX" collations into
pg_collation, rather than one per encoding. Make these instances exist
and do something useful even in machines without locale_t support: to wit,
it's now possible to force comparisons and case-folding functions to use C
locale in an otherwise non-C database, whether or not the platform has
support for using any additional collations.
Fix up severely broken upper/lower/initcap functions, too: the C/POSIX
fastpath now does what it is supposed to, and non-default collations are
handled correctly in single-byte database encodings.
Merge the two separate collation hashtables that were being maintained in
pg_locale.c, and be more wary of the possibility that we fail partway
through filling a cache entry.
Use collencoding = -1 to represent such a collation in pg_collation.
We need this to make the "default" entry work sanely, and a later
patch will fix the C/POSIX entries to be represented this way instead
of duplicating them across all encodings. All lookup operations now
search first for an entry that's database-encoding-specific, and then
for the same name with collencoding = -1.
