Peter Eisentraut 3c49c6facb Convert documentation to DocBook XML
Since some preparation work had already been done, the only source
changes left were changing empty-element tags like <xref linkend="foo">
to <xref linkend="foo"/>, and changing the DOCTYPE.

The source files are still named *.sgml, but they are actually XML files
now.  Renaming could be considered later.

In the build system, the intermediate step to convert from SGML to XML
is removed.  Everything is build straight from the source files again.
The OpenSP (or the old SP) package is no longer needed.

The documentation toolchain instructions are updated and are much
simpler now.

Peter Eisentraut 1ff01b3902 Convert SGML IDs to lower case
IDs in SGML are case insensitive, and we have accumulated a mix of upper
and lower case IDs, including different variants of the same ID.  In
XML, these will be case sensitive, so we need to fix up those
differences.  Going to all lower case seems most straightforward, and
the current build process already makes all anchors and lower case
anyway during the SGML->XML conversion, so this doesn't create any
difference in the output right now.  A future XML-only build process
would, however, maintain any mixed case ID spellings in the output, so
that is another reason to clean this up beforehand.

Peter Eisentraut c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

Peter Eisentraut bb4eefe7bf doc: Improve DocBook XML validity
DocBook XML is superficially compatible with DocBook SGML but has a
slightly stricter DTD that we have been violating in a few cases.
Although XSLT doesn't care whether the document is valid, the style
sheets don't necessarily process invalid documents correctly, so we need
to work toward fixing this.

This first commit moves the indexterms in refentry elements to an
allowed position.  It has no impact on the output.

Tom Lane c2b51cf190 Improve documentation about usage of FDW validator functions.
SGML documentation, as well as code comments, failed to note that an FDW's
validator will be applied to foreign-table options for foreign tables using
the FDW.

Tom Lane 21734d2fb8 Support writable foreign tables.
This patch adds the core-system infrastructure needed to support updates
on foreign tables, and extends contrib/postgres_fdw to allow updates
against remote Postgres servers.  There's still a great deal of room for
improvement in optimization of remote updates, but at least there's basic
functionality there now.

KaiGai Kohei, reviewed by Alexander Korotkov and Laurenz Albe, and rather
heavily revised by Tom Lane.

Tom Lane d0d75c4022 Add postgres_fdw contrib module.
There's still a lot of room for improvement, but it basically works,
and we need this to be present before we can do anything much with the
writable-foreign-tables patch.  So let's commit it and get on with testing.

Peter Eisentraut 6ef2448796 Fix a whitespace issue with the man pages
There is what may actually be a mistake in our markup.  The problem is
in a situation like

<para>
 <command>FOO</command> is ...

there is strictly speaking a line break before "FOO".  In the HTML
output, this does not appear to be a problem, but in the man page
output, this shows up, so you get double blank lines at odd places.

So far, we have attempted to work around this with an XSL hack, but
that causes other problems, such as creating run-ins in places like

<acronym>SQL</acronym> <command>COPY</command>

So fix the problem properly by removing the extra whitespace.  I only
fixed the problems that affect the man page output, not all the
places.

Tom Lane bb74240794 Implement an API to let foreign-data wrappers actually be functional.
This commit provides the core code and documentation needed.  A contrib
module test case will follow shortly.

Tom Lane 327e025071 Create the catalog infrastructure for foreign-data-wrapper handlers.
Add a fdwhandler column to pg_foreign_data_wrapper, plus HANDLER options
in the CREATE FOREIGN DATA WRAPPER and ALTER FOREIGN DATA WRAPPER commands,
plus pg_dump support for same.  Also invent a new pseudotype fdw_handler
with properties similar to language_handler.

This is split out of the "FDW API" patch for ease of review; it's all stuff
we will certainly need, regardless of any other details of the FDW API.
FDW handler functions will not actually get called yet.

In passing, fix some omissions and infelicities in foreigncmds.c.

Peter Eisentraut 6dcce3985b Remove unnecessary xref endterm attributes and title ids
The endterm attribute is mainly useful when the toolchain does not support
automatic link target text generation for a particular situation.  In  the
past, this was required by the man page tools for all reference page links,
but that is no longer the case, and it now actually gets in the way of
proper automatic link text generation.  The only remaining use cases are
currently xrefs to refsects.

Heikki Linnakangas 4e766f2d17 Always pass catalog id to the options validator function specified in
CREATE FOREIGN DATA WRAPPER. Arguably it wasn't a bug because the
documentation said that it's passed the catalog ID or zero, but surely
we should provide it when it's known. And there isn't currently any
scenario where it's not known, and I can't imagine having one in the
future either, so better remove the "or zero" escape hatch and always
pass a valid catalog ID. Backpatch to 8.4.

Peter Eisentraut 7babccb915 Add the possibility to specify an explicit validator function for foreign-data
wrappers (similar to procedural languages).  This way we don't need to retain
the nearly empty libraries, and we are more free in how to implement the
wrapper API in the future.

Peter Eisentraut cae565e503 SQL/MED catalog manipulation facilities
This doesn't do any remote or external things yet, but it gives modules
like plproxy and dblink a standardized and future-proof system for
managing their connection information.
