Command synopses using <cmdsynopsis> with multiple variants previously used
<sbr> to break lines between variants. The new man page toolchain introduced
in 9.0 makes a mess out of that, and that markup was probably wrong all along,
because <sbr> is supposed to break lines within a synopsis, not between them.
So fix that by using multiple <cmdsynopsis> elements inside <refsynopsisdiv>.

The endterm attribute is mainly useful when the toolchain does not support
automatic link target text generation for a particular situation. In the
past, this was required by the man page tools for all reference page links,
but that is no longer the case, and it now actually gets in the way of
proper automatic link text generation. The only remaining use cases are
currently xrefs to refsects.
(Possibly release notes material, lest users be confused.)
The --quiet option is now obsolete and without effect in createdb,
createuser, dropdb, dropuser; kept for compatibility but marked for
removal in 8.4.
Progress messages when acting on all databases now go to stdout instead
of stderr, since they are not in fact errors.
Ordered options in reindexdb reference page alphabetically, like in
other programs' pages.
