Daniel Gustafsson 52b49b796c Doc: Update ulinks to RFC documents to avoid redirect
The tools.ietf.org site has been decommissioned and replaced by a
number of sites serving various purposes.  Links to RFCs and BCPs
are now 301 redirected to their new respective IETF sites.  Since
this serves no purpose and only adds network overhead, update our
links to the new locations.

Backpatch to all supported versions.

Daniel Gustafsson 68387f9bdc doc: Add acronym and glossary term for Access Method
AM was used throughout the documentation to denote Access Method, but
the acronym was not described.  This adds an acronym entry as well as
a glossary term which the acronym links to.  Each page which describe
AMs have the first occurrence with <acronym> markup.

Peter Eisentraut 919c486a27 doc: New glossary entries
Add:

- "Restartpoint"
- "Log sequence number"

"LSN" was already listed in the Acronyms appendix, but it is more
suitable as a glossary entry, so move it there and have the acronyms
entry link into the glossary.

Also turn on DocBook parameter glossentry.show.acronym to show
acronyms for glossary entries, which is being used here.

Daniel Gustafsson 91199dd281 doc: Reword unexplained abbreviation
The previous wording used MVF to indicate the Most Common Values'
Frequencies, but the abbreviation was never explained or defined.
Reword to mcv_freqs to make the use clearer.

Also add MCF and MCV as acronyms as they were using <acronym>
markup but were missing from the acronyms page.

Michael Paquier 15ff5401d1 doc: Add acronyms for MITM and SNI
This adds MITM and SNI as acronyms, as the documentation already had
them marked up with <acronym>.

While on it, make sure to spell man-in-the-middle with dashes
consistently, and add acronyms for those new terms where appropriate.

Peter Eisentraut a06e56b247 doc: Update redirecting links
Update links that resulted in redirects.  Most are changes from http to
https, but there are also some other minor edits.  (There are still some
redirects where the target URL looks less elegant than the one we
currently have.  I have left those as is.)

Michael Paquier dad335b89f Replace search.cpan.org with metacpan.org
search.cpan.org has been EOL'd, with metacpan.org being the official
replacement to which URLs now redirect.  Update links to match the new
URL. Also update links to CPAN to use https as it will redirect from
http.

Andres Freund e6c039d13e Add documentation for the JIT feature.
As promised in earlier commits, this adds documentation about the new
build options, the new GUCs, about the planner logic when JIT is used,
and the benefits of JIT in general.

Also adds a more implementation oriented README.

I'm sure we're going to want to expand this further, but I think this
is a reasonable start.

Peter Eisentraut 1ff01b3902 Convert SGML IDs to lower case
IDs in SGML are case insensitive, and we have accumulated a mix of upper
and lower case IDs, including different variants of the same ID.  In
XML, these will be case sensitive, so we need to fix up those
differences.  Going to all lower case seems most straightforward, and
the current build process already makes all anchors and lower case
anyway during the SGML->XML conversion, so this doesn't create any
difference in the output right now.  A future XML-only build process
would, however, maintain any mixed case ID spellings in the output, so
that is another reason to clean this up beforehand.

Peter Eisentraut c29c578908 Don't use SGML empty tags
For DocBook XML compatibility, don't use SGML empty tags (</>) anymore,
replace by the full tag name.  Add a warning option to catch future
occurrences.

Tom Lane 7f77cbd996 Change documentation references to PG website to use https: not http:
This is more secure, and saves a redirect since we no longer accept
plain HTTP connections on the website.

References in code comments should probably be updated too, but
that doesn't seem to need back-patching, whereas this does.

Also, in the 9.2 branch, remove suggestion that you can get the
source code via FTP, since that service will be shut down soon.

Tom Lane 8daeb5ddd6 Add SP-GiST (space-partitioned GiST) index access method.
SP-GiST is comparable to GiST in flexibility, but supports non-balanced
partitioned search structures rather than balanced trees.  As described at
PGCon 2011, this new indexing structure can beat GiST in both index build
time and query speed for search problems that it is well matched to.

There are a number of areas that could still use improvement, but at this
point the code seems committable.

Tom Lane d9572c4e3b Core support for "extensions", which are packages of SQL objects.
This patch adds the server infrastructure to support extensions.
There is still one significant loose end, namely how to make it play nice
with pg_upgrade, so I am not yet committing the changes that would make
all the contrib modules depend on this feature.

In passing, fix a disturbingly large amount of breakage in
AlterObjectNamespace() and callers.

Heikki Linnakangas 15c121b3ed Rewrite the FSM. Instead of relying on a fixed-size shared memory segment, the
free space information is stored in a dedicated FSM relation fork, with each
relation (except for hash indexes; they don't use FSM).

This eliminates the max_fsm_relations and max_fsm_pages GUC options; remove any
trace of them from the backend, initdb, and documentation.

Rewrite contrib/pg_freespacemap to match the new FSM implementation. Also
introduce a new variant of the get_raw_page(regclass, int4, int4) function in
contrib/pageinspect that let's you to return pages from any relation fork, and
a new fsm_page_contents() function to inspect the new FSM pages.
